Case Manager Resource File - #1

◼
In this assignment you are to search the local, state, regional,
international resources and websites related to supports,
organizations, businesses, technology, AT devices, and Internet
resources for persons with disabilities in various domains as
outlined in this syllabus and in the next slide.
Because people will often have questions about where to go for
various resources you are to become more knowledgeable and
informed regarding what those resources are. Therefore, you are to
construct a supply of resources that may be useful for people living
with the specific disability chosen by the student.
Students have a choice about what disability group they will focus
on as I want this project to be personally meaningful and in-line
with one group type of disability that is really of interest to you as
a future RC professional.

Your completed project will consist of 30 resources with descriptions.
If you are using a website, please give the spelled out name/title of the
website along with the link and a 1/3 to ½ single spaced typed
narration of what is located on that website. Assignment is due
November 20th at 5 pm.
Resources may be local, state, regional, national or international but
they need to be directly related to the disability type (i.e., TBI, SCI, Bi
Polar, Schizophrenia, Dyslexia etc.). If it is not an organization that
specifically serves this population and instead works with a variety of
people with disabilities, please explain in your narrative how this
organization/support can be useful for people with this specific
disability type.
The goal is to have a number of resources with information you can
draw upon and access when you graduate or go out into your
practicums and internships. An essential component of being an
effective rehabilitation counseling professional is to know what
resources and supports are out there even if they are beyond what you
do in your day-to-day work. Sometimes we may find ourselves in a
position to help guide and educate people about resources they can
access to help improve their overall quality of life.

CASE MANAGEMENT RESOURCE FILE
Students are to pick a disability group and assemble a resource binder to better serve the chosen
disability group (i.e. spinal cord injury, TBI, developmental disability). Resource binder must be
a comprehensive source of information for individuals with disabilities. Resources may be found
in the community, online, or include national resources.
Your task is to determine what the specific needs might be for an individual with this specific
disability type. You are to become a specialist of knowledge for the given disability type. For
example, if you choose spinal cord injury, try to envision what areas are important for an individual
to become a well-integrated member of society. What are his or her needs: interpersonally,
recreationally, medically, psychologically, and relationally (i.e., family, sexually), vocationally
(i.e. accommodations)?
In other words, if you were to serve individuals with a given disability try to envision you are the
‘go to professional’ within the community; therefore, it’s your job to scout resources and learn
as much as you can about availability to assist you with case management and provision of services
and resources to this specific group of individuals with your targeted disability population. Please
note the resources must be specifically related to the disability type chosen and to be of use to help
them overcome barriers, and successfully integrate and be a part of the community. Ideally, it
would be best to have a crate full of resources, but since this is an online course, I am going to ask
for a booklet/manual of at least 30 relevant and applicable resources along with descriptions.
Sample areas and categories may include some of the following:
A. Disability Legislation and the ADA
B. Personal Care/Personal Independence & Functioning
C. Education
D. Work/Employment
E. Recreation and Leisure
F. Adjustment to Disability
G. Family Coping Resources
H. Mobility Impairments
I. Communication & Sensory Impairments
J. Prosthetics & Orthotics
K. Clinical Treatment & Psychotherapy
L. Assistive Technology
Thus, students are to choose a specific type of disability, consider the ways this disability affects
their lives, reflect on the numerous parts of a person’s life (i.e., personal, social, recreational,
vocational, medical, spiritual, etc.), and research local, state, regional, or national resources,
agencies, or products that would help someone with this disability type. Students may also want
to consider assistive technology (AT) needs and devices.
As a part of this assignment, students are to divide resources into life domains or categories as
listed above, give the name of resource, contact information [i.e., address, phone, website], and a
summary of what this resource does or can be used for by the person with disability. Each written
resource summary should be 1/3 to 1/2 page long, single-spaced.
Students’ grade will be based on the following:
A. Title Page, Table of Contents with Page Numbers and Outline of Categories (10 pts)
C. Grammar, Writing Style, Legibility: (10 pts)
D. Completion of Assignment Requirements (At least 30 resources spread among 6 or more
categories and 1/3 to 1 /2 page summary of the content/resources available from this resource) (60
pts – 2 pts each resource and explanation)
Case management resource file is worth a total of 80 points and is due on Blackboard,
November 20th at 5 pm.
