Question description

The explosion at Imperial Sugar was a tragedy, made more heartbreaking by the fact that the US Chemical Safety Board investigation determined that the accident was completely preventable with better workplace practices.  There seem to be a lot of potential ways for these problems to have been addressed before the accident.  Please spend a little time thinking about how the following people might have prevented the accident and why that didn't happen.

Employees working in that facility

state regulators

federal agencies

What conditions might have prevented these individuals from acting?  Is there any group in particular that you believe should have acted differently?  If you were an executive with the company, a manager in that facility, or an employee in that facility, what would you have done?  Please post your response.
