From the picture it looks like the slope of the given line is

8 - 0 / 2-(-1) = 8/3

Therefore the slope of the perpendicular line is the negative reciprocal, which is -3/8

Now we use the point/slope formula to solve for the given point (4,8)

y - 8 = -3/8(x - 4)

y - 8 = -3/8x + 3/2

y = -3/8x + 3/2 + 8

y = -3/8x + 19/2

On the worksheet i counted up 12  & over 4

I couldn't see the first point from the picture you provided. So I went with where the line crosses the x-axis.

But, if you're sure it's 12/4 then it's even easier because the slope is 3

So, the slope of the perpendicular line -1/3

Then,

y - 8 = -1/3(x - 4)

y - 8 = -1/3x + 4/3

y = -1/3x + 4/3 + 8

y = -1/3x + 28/3
