a) Challenges
Firstly patient Acuity has been growing patient acuity in the LTC centers as there are two
groups of patients with different needs. The first groups are the residents who live within the
healthcare settings due to care needs and patients who are recovering from specific illnesses and
they stay for a few weeks. Nursing homes are facing large numbers of patients who are in need
of living assistance. The challenge is in preserving the quality of life and health for the long term
patients.
Secondly, registration data gaps exist when capturing patients’ data. The data recording is
difficult as patients may forget details and their helpers may not be near. Therefore there is a
need to link the patient's details in LTC to the HER system to ensure a more sustainable tracking
of patients and their needs and make adequate arrangements in advance.
Thirdly shortage of Clinical Staff is another challenge. There is a need for geriatric nursing
which is based on care that promotes a patient's independence and ensuring that their quality of
life is maintained throug...
