Question Description

Please make a social work case presentation after looking at case notes and there is a power point which is not done but has some ideas and information. Presentation have to be 15 min long so include that much that would last 15 minutes( plz make power point ) From the case notes you will find information regarding resources in community and how social worker is working with in collaboration with other agencies naming few such eg. Case presentation of Mr. john https://youtu.be/wlBYCl2Ytyo Hoarding Behavior What is hoarding behavior • Holding lager number of items that most people do not consider them useful. • Cluttered living spaces • Inability to discard items • Keeping stacks of newspapers, magazines, or junk mail • Moving items from one pile to another without discarding anything • Acquiring unneeded or seemingly useless items, including trash • Difficulty managing daily activities, procrastinating and trouble making decisions • Difficulty organizing items • Excessive attachment to possessions and discomfort letting others touch or borrow possessions • Limited or no social interactions. Scenario • • • • • • • • • Outreach worker got referral from building manager that Mr. John has cluttered sweet which is a safety issue. An outreach worker visited him with his consent. His sweet was packed up with stuff that there is no pathway to move around. Baseboard heaters are covered with stakes of stuff. Kitchen shelves has no space left. Living room was full of stuff packed up to the roof. Observation by social worker- Client at risk of fall. He was wobbling while walking since there were no pathways to move around. Client had also mentioned that he is having frequent falls. Client mentioned to worker that he is felling disoriented. Client is given with a letter from building manager to declutter his sweet in one month time frame. Client's stove is unplugged because of safety issues. Bed bug inspection is going to put in place when sweet is decluttered. At another visit social worker introduce him to START program and set up some small goals with him. Social worker connect Mr. john to compassion network for house keeping services. Client’s sweet started looking better day by day. It seems his confidence is coming back and he is happy about his progress. Client does not want to throw or donate stuff through compassion network, it seems he just moved things around to make is look nice. Mr. john kept cancelling his appointments with social worker and other agencies involved in care. Social worker could get hold of him after few weeks and during visit he mentioned that he is loosing hope in life. continued------ Scenario continued… • Client is connected with START program at Glenrose where he is getting • • • • Client is being reported of having falls frequently. Client had periods of going back and forth to cluttering. Clients sweet getting dirtier Social worker noticed that there are hazards in his sweet such as blender sitting on top of stove melted and stick to it. Client was offered by social if he would consider living in lounge. Client agreed and had a tour of lounge. Client s on the list to move to lounge and getting rid of his stuff and he is figuring out where he is going to store his left over stuff. Client is planning to donate some of his stuff and on the other hand client is worried that he would not be able to cook in the lounge. Client is referred to apply for new walker because of safety concerns. Client was talking about buying some thing to take to the lounge with him. Client is expressing suicidal ideas to social worker upon next visit. Client is connected with START program again. Met with john in his suite. johns's livingroom was less full, he had a clear pathway, and his garbage and recycling were empty. Writer congratulated him on his hard work. He said at the Glenrose the treatment involves everything mental health, physical health,... He said he has been enjoying having The Compassion Network come to clean. Received a voicemail from John saying he just got back from a clinic and they gave 0 him 3 injections. He said these have made him tired and he will need to postpone our meeting for today. John phoned asking for me to call Service Canada today to get his GIS fast tracked. I let 0 him know that I cannot phone without him there. We booked an appointment for me to see him on Tuesday, April 10th at 1:00. Emailed Colleen Derkson updating her on my appointment with Frank and asking her if 0 she has seen John recently. Updated Sari on John's suite. She said she saw it for his inspections and agreed it is 0 getting bad again. (4) John said he would like to get his taxes done. I let him know colleen told him she 0 would book an appointment for him, but I will double check to see if she did. I let her know I will check to see when she can see him next. (3) John said that he does not always feel life is worth living, and he said the only reason he will not end his life is because he does not want bad karma, and become an 0 insect in his next life. John said he has nothing meaningful in his life, but as we listed off supports and activities in his life he agreed that they were meaningful and enjoyable. He agreed I can come see him again in a couple of weeks. (2) I reminded John that he needs to keep the entrance way and the path cleared so that he has no tripping hazards. He agreed that he needs to keep it safe. He said he has still not received his GIS, so I tried to phone Service Canada for him but the line was too 0 busy to take my call. He said he has been going to the Glenrose for exercises and it is going well. He said he goes twice a week for the next 6 months. Shannon filled out the Background Survey with Frank for Q of L. I smelled of old pop cans, and his kitchen was dirty and messy. He said he has 0 been taking out a box a day, and that The Compassion Network is coming later this week. He said Colleen has not come to see him recently, and they have made no new goals.
