Why should a teacher make an analysis of the results of an administered test and its questions? What is the best tool for determining which items/item responses should be revised? Why?

An analysis of the results enables a teacher to gauge the overall performance of the class, performance in certain questions and topics, determine top performers poor performers, improvements etc.In short analysis of results enables a teacher to carry out a variety of determinations.

The best tool is to determine the average performance per topic or question. Topics or questions with low averages indicate that many of the  students did not understand the topic or question and should thus be revised
