At this stage of the project, it will be a two-step approach: investigating the data obtained for the various input and target variables and the data cleansing to account for incorrect or missing values (Chapman, Clinton, Kerber, Khabaza, Reinartz, Reinarts, Shearer, and Wirth., 2000). Business Problem With the use of a more comprehensive field triage model to predict the patient’s status at hospital discharge, the management team at corporate wants to reduce burn patient costs by 15% by June 2021 that will include a minimum of a 10% reduction of the number of 30-day readmissions. The current practice today is to consider the Baux Score, “a comparative indicator of burn severity, with a score over 140 considered as being un-survivable, depending on the available treatment resources” which is determined by the total burn surface area and age of the patient (Osler, T; Glance, LG; Hosmer, 2010). The total burn surface area is based upon the Wallace Rule of Nines (Osler, et al., 2010). Analytics Problem Statement The analysis will include the comparison of four models: logistic regression, CHAID, discriminant analysis and neural network to determine the highest ranked predictive model for field diagnostic used to predict the patient’s status at the time of discharge. The relationship between the predictor variables and the target variable will be investigated using SPSS and Microsoft Excel. The highest ranked model will consider a minimum model accuracy requirement of 70% and a consider any over-fitting model tendencies, standard business DATA NEEDS INDENTIFICATION AND DATA ACQUISITION 3 requirements. Factors that affect burn patient status include age, gender, race, total burn surface area based on Wallace Rule of Nines, inhalation injury and flame injury. (Osler, et. Al, 2000). 1000 HIPPA-compliant records for the dataset are sourced from the health system’s ERP by the Chief Data Scientist, Ken Ferrell. (Ferrell, 2018). Analytics Dataset The burn patient dataset was provided in a text file that was collected from 40 medical facilities in the health care system for educational purposes. The historical data is pre-scrubbed from patient identification prior to data removal from the ERP to be HIPPA compliant. The facility identification data is coded and is provided without the ability of the team to identify any specific medical facilities to adhere to corporate’s privacy policies. In addition to the investigation of predictor importance, the relationship between the predictor variables, factors that influence burn severity, and the patient status at discharge, dead or alive, will be analyzed. The variables that are required to predict the burn patient status at discharge are based on the data historically collected at the first time of hospital admittance to a facility of the health care system and should not include any “within 30-day re-admission” patients. Identification of Each Variable and Its Measurement of that Data To perform the model analysis, Death (alive or dead), the status of patient at hospital discharge as reported by various facilities within the health care system, is the categorical and target output variable. Nominal data are patient status, gender, race, inhalation injury and flame injury. Cardinal data are age and total burn surface area (tbsa) (Smith, 2016). The continuous input variables are age (decimal) and total burn surface area, tbsa, (percent). The SPSS Modeler flags are gender, race, inhalation injury (inh_inj) and flame injury (flame). An inhalation injury refers to damage to the respiratory system that is caused by breathing in toxic air from smoke DATA NEEDS INDENTIFICATION AND DATA ACQUISITION 4 (Osler, et Al., 2010). A flame injury refers to severe skin damage due to fire that causes the cells in the affected area to die (Osler, et Al., 2010). There are 1000 historical, HIPPA-compliant, burn patient data provided in a text file and imported to an Excel Workbook. The role of the variables used in the diagnostic and descriptive analytics are listed in Table 1. Table 1. There were no missing data and the assumption of correct values was made based on the absence of unrealistic numbers. Table 2. illustrates that patient status was an 85/15 ratio mix, gender was 70/30 ratio mix, inhalation injury was 12/88 ratio mix and flame injury was 53/47 ratio mix. Table 2. Frequency of Age of Patient at Time of Admittance 6 DATA NEEDS INDENTIFICATION AND DATA ACQUISITION 7 Identification of Comparison Models This analysis seeks to develop a best-fit model with a minimum of a 70% accuracy that will be used to predict the burn patient’s status at time of hospital discharge. The model types are a consideration when identifying needs and data acquisition. Once the highest ranked, more comprehensive model is developed, medical staff can use it to efficiently triage burn patients based on a high level of accuracy. The data provided includes 1000 different records of burn patients and details on their age, gender, race, total body surface area, inhalation injury, flame injury and patient’s status at time of discharge. In order to come up with the best predictive model, an analysis tool such as SPSS will be used model Logistic Regression, CHAID, Discriminant Analysis, and Neural Network models. Logistic Regression will be used to create the model equation that describes the relationship of age, race, gender, inhalation injury, flame injury and total burn surface area to the patient’s status at time of hospital discharge. CHAID, or Chi-squared Automatic Interaction Detection, a model used for classification in building decision trees by using chi-square statistics to identify optimal splits will be examined. (IBM, 2019). CHAID, will be used to regress the six predictor variables against the dependent variable, that is, Death (alive or dead). The regression will provide a model that includes coefficients of all the variables which can be used to predict the dependent variable, that is, the status of the burn victims when they leave the hospital. Another model of interest is Discriminant Analysis, which is used to categorize records based on statistical techniques that is “analogous to linear regression; but takes a categorical target field instead of a numeric one”. (IBM, 2019). In addition to SPSS’s models, Tableau could also be used to create visualizations such as bar graphs, dot plots and line graphs to show how each of the independent variables effect on the status of the burn patients when they leave the hospital. A DATA NEEDS INDENTIFICATION AND DATA ACQUISITION final SPSS model that will be used for model comparison and ranking, is the Neural Net.
