Hughes notes that the desire for quality health outcome is the reason behind healthcare
improvement initiatives (2008). Hughes defines quality healthcare as health services for persons
and populations that increase the likelihood of quality health desired outcomes. The article
describes medical errors as a result of faulty processes and systems and not a result of medical
negligence. According to Hughes, when processes are variable and inefficient like health
insurance, changing patient mix, healthcare provider level of experience and education are part
of many factors the make healthcare complex (2008). The author introduces six aims of the
Institute of Medicine which can make healthcare function at a higher level resulting to quality
healthcare; the aims of healthcare are patient-centered, safe, effective, efficient, timely and
equitable. Since poor healthcare is caused by errors and system failures it is important to point
out ineffective care, inefficiencies, and preventable errors to influence quality healthcare
changes.
