Introduction and epidemiology
Diabetes mellitus is an endocrine disease that is characterized by hyperglycemia due to
relative insulin deficiency or resistance of insulin receptors. There are two main types of diabetes
mellitus: Type 1 diabetes mellitus (insulin dependent diabetes mellitus) which mainly affect the
young people with a peak age of onset being 16-25 years and type 2 diabetes mellitus (noninsulin dependent diabetes mellitus) which occurs mostly in the older and obese people (CDC,
2011). Other types of diabetes include gestational diabetes, Maturity Onset Diabetes of the
Young (MODY). According to the WHO statistics, by the year 2014, 422 million people had
diabetes, an increase from 108 million people in the year 1980 (WHO, 2016). Its incidence has
been rising in the middle- and low-income countries due to the rise in the prevalence of obesity
and sedentary lifestyle in these countries. The worldwide incidence among those above 18 years
has increased from 4.7% in the year 1980 to 8.5% in the year 2014 (WHO, 2016). In the year
2016, about 1.6 million people died due to the direct effects of diabetes (WHO, 2016). Diabetes
also contributes significantly to the occurrence of kidney failure, blindness, amputations,
coronary vascular diseases, and cerebrovascular accidents.
Etiology
The occurrence of diabetes is as a result of the interaction that exists between
environmental factors and genetics in a susceptible host. It has been shown that there is a 2-3%
chance of developing diabetes in children whose mothers had diabetes mellitus, 5-6% risk if the
father had diabetes and 30% chance if both parents had diabetes hence showing how genetics
contribute to the occurrence of the disease (Shaw et al., 2010). Environmental factors such as

viral infections, cow milk protein allergy, lack of vitamin D and UV light exposure also
contribute to the occurrence of diabetes.
