The U.S. healthcare system is influenced by a shifting political climate, constant economic development, rapidly evolving technological innovation, diffusion of social values, demanding workforce issues, and demographics trends. Also, the U.S. system differs from other countries’ systems on many dimensions, such as lack of a central governing agency, limited coordination, high costs but only average health outcomes, imperfect market conditions, the dominant private sector market, multiple organizational forms and players, and the conflict between market goals and social justice. The concern over the economic future of healthcare systems revolves around three broad issues: cost, quality, and access. Gaps in coverage, combined with the upward trends in medical care spending over the past several decades, add to the commonly held belief that the U.S. healthcare systems is in crisis.

As a final project for HCM375, prepare and submit a scholarly paper that discusses the following:

  • Describe the economic foundation of the U.S. healthcare delivery system and the role of economics in healthcare.
  • Compare and contrast the use of various economic models to explain the role of economics in healthcare and how the concept of the market equilibrium works in healthcare.
  • Assess the role of production of health and demand and supply for medical care and health insurance, and relationship of income and the demand for healthcare.
  • Compare the economic benefits and challenges of individual versus population health.

Portfolio Project Option #2: Competitive Markets and Market Power

Assume you are the Chief Executive Officer of a large and complex integrated health delivery system (IHDS). IHDS is a for-profit health system and is located in a major metropolitan area market in the United States. The metropolitan market is highly competitive, with multiple and similar large integrated health delivery systems (private and public) that compete directly with IHDS. Some of the characteristics of this market are booming economic activities, a dynamic and diverse population, labor market shortages, and a large uninsured population (25% of the total population), which has been has been trending higher.

IHDS operates multiple long-term acute care facilities and affiliated teaching hospitals, outpatient and surgical centers, rehab and ancillary facilities, wellbeing and urgent care outlets, and multiple physician group practices (primary care and subspecialties clinics in different locations). IHDS also has affiliation agreements with regional and national managed care plans and third-party payers. Also, IHDS has an international medical facility that caters to an international clientele who seek advanced medical treatment in the United States

The recent healthcare reforms (Affordable Care Act, Accountable Care Organizations, shift from volume-based to value-based healthcare delivery systems, bundled payment system, etc.) are starting to impact the economic model on both macro- and micro-levels as well as the competitive advantage current position and outlook for IHDS. The Board of Directors (BOD) has asked you to prepare an executive report about the economic impact of these trends and forces mentioned above and to assess the economic outlook for your organization.

As final project for HCM375, prepare and submit an executive report that discusses the following:

  • Assess the economic assumptions of a competitive market model on IHDS and identify the evidence for and against the competitive model.
  • Evaluate the potential economic consequences of the applied community rating and experience rating on IHDS.
  • Compare and contrast alternative delivery arrangements for IHDS in terms of their micro economic efficiency.
  • Describe the determinants of market power for IHDS and assess how IHDS can use the market power to demonstrate superior economic efficiency and effectiveness. Explain how market power can affect labor market outcomes for your organization.

Describe the economic foundation of the U.S. healthcare delivery system and the role of the
economics in healthcare.
Introduction
The U.S. healthcare delivery system is made up of divided network of private and public
financing, quality assurance structures, and health care delivery. 58% of the hospitals in the
country are non-profit, 21% for-profit, and 21% are government owned. The U.S. does not have
a national entity or set policies which can be used to guide the health care system. Those
operating in the private sector are either profit-making entities or non-profit making entities.
Regulators also control the health sector; some who serve voluntary while others are
governmental. The United States’ healthcare delivery system is faced with different cost and
quality challenges. The country is the biggest spender on healthcare per capita in the world
which was $4, 637 in 2000 reflecting $1.3 trillion. The amount was $2.5 trillion in 2009 which
was $8, 086 per person (Barr, 2011).
The health care delivery system is made up of the payers, consumers, providers, suppliers, and
regulators. The Fee-for-service (FFS) payments are the main form of reimbursement to the health
care providers. The costs are issues retrospectively to the services providers once the health
services have been provided. The FFS incentivize healthcare providers so that they can provide
more services to the people. Economics ensure that the benefits of healthcare spending are
maximized because the service providers are held responsible for their services. It also helps in
managing and controlling demand as healthcare services are made readily available to every
American (Berger, 2008). Suppliers of health care products can provide a bargaining power

because they determine the health of the patient; they are the main force which controls the
economies of American healthcare.
