The home health I went was from Alina and we visited four clients ,but we were supposed to visit five unfortunately one of them does not like to be attended by student nurse. The few we visited are as a follows (A) He was involved in a motor vehicle accident a month ago., where he had a back pain and a lower abdomen pain. he was in the hospital for a week before he came home. At home he stays with his wife who helps him with daily activities especially taking his medication and going to the doctor’s appointment. he has two nurses who visit him two times a week. the nurses get to check his INR and his pain level. He also has a physical therapy who comes two times a week to work with him. (B) The second patient we saw was a cancer patient. She was going through chemotherapy at home. she had a pic line and a central line plus two tubes coming from her kidneys. the nurse was able to change the pic line and the central line dressings using a sterile technique. She also lives with her husband and his two sons and their wives. she goes to the clinic once a week to see her primary doctor. (C) The third patient we visited she had bilateral knee replacement a week ago. She lives in a farm with her husband who helps her with her daily activities .she was on pain medication every for hours .that day she stated that her pain level has decreased to 4 on a pain scale of 0 to 10 (D) The last client we saw was a widow lady who was 84 years old. she had cellulitis on her left leg that lead into getting surgery on her leg. She lives alone but her kids visit her everyday since she got sick. The nurses had to go see her three times a week because she needed a dressing change. (2) Evaluate how respect for ethnic identity and socio-cultural diversity is conveyed in this setting. (3) Describe how infection control principles are utilized at this community setting. The nurse was very good at that, she had her own gloves and hand sanitizer. before she goes to any client’s home she had to sanitize her hands. after using her vital equipment’s, she had to wipe them with alcohol wipes. she also had her own drape that she used to place her vital equipment’s, she had each for each house, meaning if she uses one at one home she throws it away . I felt that she was on top on infection control. For example at one home where she had to do a INR check she had to wipe the INR machine before she used it on the client, she also wiped it off again after she used it. (4) Describe how the registered nurse functions as a provider of care, manager of population groups and case management within the discipline of nursing in the community setting. (5) Determine potential health education needs based on your assessment of this client/family/community. (6) Explain how the information found in a current (in the last 4 years) nursing journal could be utilized in the community setting you observed. (7) Analyze your feelings and attitudes about providing nursing care in the community setting.
