I don't understand how to write a lab report for chemistry?

My chemistry teacher gave a lab report due tomorrow with no explanation how to do it and it is our first one of the year. She wants us to start off with Results and Analysis and then Calculations. How would I write this? In paragraph form or in tables?

Title: In a few words give a title to your report, you can get this from lab manual

Introduction: Tell in brief what the lab is about. Normally you can get this from lab manual if you have any

Procedure: Now tell the procedure followed. Copy this from manual. Do not copy word for word. Just summarize

Results: Tables and graphs are the best way to display your results. If you can make a table the better. Explain the results, say for example" From the table it is clear that.....",

Conclusions: Provide a summary of the results
