One of the mistakes that was made in the scenario is entry on the wrong phone number in
Mr. Steven’s health records. If the phone number had not been erroneous entered into the system,
Denise would not have revealed Mr. Steven’s health information to the family. The manager
decision to review all the records for such errors was appropriate and necessary to prevent a
recurrence. The mistakes points to lack of sufficient training for the staff involve in data entry.
