For your Final Assignment, you will demonstrate your knowledge of psychological assessment by applying the information you have learned throughout this course in the interpretation and write up of a psychological assessment report. Your Final Project will be based on one of the case information/data tables that have been provided in the course. The three cases consist of one adolescent assessment, one adult assessment, and one geriatric assessment. It is expected that your Week Five final project case will be the same case that you selected in Week Two and that you incorporate feedback provided to you by your instructor on the Week Two assignment when developing your final project. To complete this assignment, you will choose the client from the list below which you chose for your Week Two assignment.

As you write up your assessment report you will be taking on the role of a clinician who is conducting an assessment and providing treatment recommendations for the client that you choose from the list provided. You must use the information provided in case history and identify the most salient information that belongs in each section. Do not simply copy and paste the information provided. You must make a professional judgment about which information is the most important information to include in the psychological report and where to include that information in your report. Your assessment report must follow the format below and it must include each of the sections and their headings listed below:

I. Identifying Information
Within this section, you will record basic information on your client including the person’s name, sex, gender, ethnicity, sexual orientation, age, handedness, and occupation or grade level. For the purposes of this assignment, you are free to create any relevant demographic information that is not explicitly stated in the case scenario. All information you create must be consistent with the information provided and any conclusions you draw in subsequent sections of your paper.

II. Reason for Referral
Within this section describe the referral source and the purpose of the assessment. The information you provide in this section must justify the decision to conduct a formal psychological assessment based and must model ethically and professionally responsible assessment practices.

III. Current Symptoms/Presenting Concerns
The information in this section of the report would typically come from an interview with the client and family (if applicable, e.g., if the client is a child or person with suspected dementia). You must use the information provided in case history to identify the most salient information that belongs in this section. Choose information to include in this section based on the consistency with the reason for referral and purpose for testing. Here is where you will apply your methodological and theoretical assessment formulations of the client that will justify the decision to conduct a psychological evaluation on this client.

IV. Psychosocial History (complete each of the sections below based on the information in the case you selected)

  • Medical history (including substance use/abuse)

V. Interpretation of the Results
In this section explain your interpretation of the results in the data table provided for the case you selected. Utilize the information available and create appropriate subheadings to organize the results. For example, if your data table contains information on intelligence and achievement, then you should create appropriate subheadings to organize your findings in this section of the report. Create a sufficient number of subheadings to allow you to provide interpretations for all assessment instruments administered. If you have more than one measure of a particular psychological construct (e.g., personality and emotional functioning), present your interpretations of all measures that apply to that construct under the same subheading.

VI. Diagnostic Impressions
Based on the history provided and interpretation of test results, use the DSM-5 to provide a diagnosis (or diagnoses) for the client in a manner that demonstrates the ethical and professional use of assessment results. You must justify your diagnostic conclusions based on your knowledge of the validity and reliability of the assessment instruments. If there are multiple potential diagnoses to consider, then these must be explained and justified as well. Also include information about alternative diagnoses and why these were not chosen.

VII. Recommendations
Within this section, provide treatment recommendations for the client based on the diagnosis and information about the client’s current living situation. Develop recommendations that are evidence-based and include peer-reviewed articles that support your choice(s).

Identifying information The patient I will be evaluating is Timothy Childers, he was born and raised in Minnesota and is 15 years old in the 10th grade and he is right-handed. Reason for referral Timothy is being referred for a neuropsychological evaluation because his parents and his teacher, based off a school report, seem to think that there could be a potential learning disability. “In general, the process of assessment begins with a referral for assessment from a source such as a teacher, school psychologist, counselor, judge, clinician or corporate human resources specialist.” Cohen, R. J., Swerdlik, M. E., & Sturman, E. D. (2018) His mother first noticed that there was an issue with Timothy’s academics and behavior while he was in the 1st grade. His mother noticed that he seemed to struggle keeping focus or staying on task, that he was having issues with vocabulary and grammar, and issues doing things that involve more than one task because he will sometimes miss a very important step in completing his process. His mom noticed that while never really having issues with his math, his teachers seems to have an issue due to the fact that his work would be correct but Timothy would not showing how he is solved the problems which has caused doubt in his teachers’ mind about how honest Timothy is being in his work to get those answers. There also was the fact that Timothy also struggled with grammar and vocabulary and his focus all seemed to be off as which which is turn is causing Timothy to miss important steps in projects. III. Current Symptoms/Presenting Concerns After evaluating Timothy and his mother and using the information provided in his school interpretative report there seemed to be several concerns with the way Timothy behaves at school and at home. One of those issues was the way that he completes tasks, he sometimes forgets important steps in the tasks which causes the whole task to fail and not get completed properly. According to his mother Timothy has been pretty average in school for his age. There were some issues that his mother felt he had when it came to Timothy’s focus and how he behaves by acting on impulse. His mother also indicated that she sees Timothy as having difficulty with organizing things like his room or his school work and also does not feel that he does will with sequencing steps needed to achieve a goal, that being said Timothy seems to do well at Math and does well with numbers he just doesn’t show his work because it involves him having to sequence the problems into step by step instructions which seems to be an issue for Timothy since the 1st grade. The concern that Timothy doesn’t seem to do well with focus and is not really good at sequencing and organizing could be equated to his age, but it could also be equated to the fact that Timothy is may have an attention deficit issue. After completing the MMPI-A clinical profile there was indication that Timothy had serious behavior problems to include: school maladjustment, family discord, and authority conflicts. The profile indicated that there were times where Timothy’s poor judgement seemed to affect his behavior as well and caused him to get into trouble. Also, after getting into trouble Timothy seemed to show little remorse for his actions and would run away or lie to avoid getting in trouble. Based off of these concerns there seems to be ample reason to check Timothy out and have him evaluated for some type of behavioral issue or learning disability. Although having all of this information from his school report is very useful more testing would be required to accurately assess Timothy and make the diagnosis of whether or not he has ADHD or some other form of learning disability. “Individuals are typically able to falsify self-report behavioral rating scales, one of the primary methods of assessing ADHD.” Courrégé, S. C., Skeel, R. L., Feder, A. H., & Boress, K. S. (2019). Even more reason why going off of the accounts from Timothy and his mother are not enough and why tests need to be conducted to determine if there is a deeper issue occurring. IV. Psychosocial History Timothy’s psychosocial history with regards to his education is that he seems to have quite a feel issues with school. The issues arise when his math teacher instructs him to show his work to prove that he is not cheating on his assignments, and when asked to do things that involve spelling or grammar he seems to not want to give that area any attention. Timothy appears to have issues with being argumentative and rebellious with his teachers and his peers due to the nature of his self-centered ways. Although Timothy claimed to not have any substance abuse issues there did seem to be an issue with that from his school’s assessment. There seemed to be a high school on the MAC-R that scale that suggests a problem with alcohol or drug use which would prove that Timothy uses lies to try and get himself out of trouble if he does have an issue with substance abuse. There was not much evidence provided but it was advised to do further research into this problem. Although these results showed this information, Timothy and his mother both felt that there were no psychiatric or substance abuse history for him. As far as Timothy’s social history. According to the test done by his school Timothy seemed likeable and could make a good impression on others but it was stated that his relationships tended to be troubled. Despite that report from his school saying one thing his mom seems to believe that Timothy has a good circle of friends and that she has no concern with his social development.
