Create a personal portfolio of your experiences in the practicum. All portfolio items must be submitted; therefore scan any paper items that may not be electronically stored. The portfolio should include:
- Professional goal statement.
- A counseling case study, with identifying information carefully removed, showing your counseling theoretical perspective, biopsychosocial assessment, a collaboratively constructed treatment plan, course of treatment, and discharge summary.
- Presentations within the agency setting or at conferences or professional meetings.
- Personal bibliography of books and articles read during your program.
APA style is not required, but solid academic writing is expected.

Professional goal statement
Growing up, I had always wanted to pursue a career in the field of counselling due to my interest
in helping others live a fulfilling happy life. Growing up, I saw a number of people struggle with
various psychological issues especially depression and drug abuse among the youths. But my
interest in pursuing a career in counselling became more pronounced after a number of my
friends dropped out of school while others were arrested for illegal drug use. By the time I
graduated from college, I was sure that I wanted to do something that will address the significant
issues of drug and substance use that I have witnessed in my community while growing up. After
completing my Master’s program, I wish to specialize in working with youths struggling with
drug and substance abuse as well as those in contact with contact with the criminal justice
system. This interest is reflected in my choice of the practicum facility that I enrolled in.
Over the years, the US criminal justice system has been interpreted more as a criminal issue
