Medication errors occur for a host of reasons and may vary from facility to facility. Common medication errors consist of omitting the 5 rights of medication administration, distractions, sound alike-look alike drugs, fatigue, and lack of pharmacological knowledge. Therefore, the cause-and-effect diagram will be most effective in reducing medication errors because it identifies the problem and the reasons they occur. After constructing the cause-and-effect diagram, solutions should also accompany the identified problems. Root cause analysis is the process of identifying a problem and finding appropriate solutions). This was the primary goal when the RCA team was constructed at Downtown Medical. The team initially got off to a bumpy start by lacking accountability. One discipline of a hospital cannot stand on its own, hence interdisciplinary collaboration. The risk manager exhibited collaboration skills from the beginning of the meeting by using words such as “our,” when she stated, “Our responsibility is to prevent it from happening again.” She steered the team in the right direction the entire meeting and eventually the team was able to reconcile their differences, collaborate, and focus on the goal at hand, eliminating medication errors.When analyzing the RCA team, more disciplines should have been involved, especially a physician.
