Each student is to find an example of something that failed (could be by deformation or fracture) at home, an automobile repair shop, a machine shop, on campus look around - take pictures and complete the following template for a "preliminary case study". This is NOT a web search for a case study. You can use the web to find explanations for your failed part or system, but you MUST be able to examine and take pictures to put in the report. Failure Analysis Report Name: Failure of plastic water bottle carrying handle Type of failure: Fatigue Description of the failure: First there was a line lighter than the rest of the carrying handle, then a small crack formed at one end of the handle. With time, the crack became larger and it kept growing until one day the carrying handle separated into two different parts. There was relatively little plastic deformation and the crack propagated in a nearly straight line, perpendicular to the applied force. Reasons of the failure: The carrying handle failed because it was misused during operation, and the root cause of this failure was human. The carrying handle was subjected to a repetitive flexural stress. Every time I would drink water because the lid of the bottle would be in my face, I would bend the carrying handle in the direction opposite to the way it was bent normally to hold down the lid of the water bottle. And even when I saw the material showing signs of fatigue, I kept applying the flexural stress until one day it snapped. Suggestion to avoid the failure: The main suggestion I can give since the failure does not come from the manufacturing process, would be to add a manual explaining how to use the water bottle and the handle and to emphasize on the fact that one should not bent the handle in the opposite direction. But also, I think manufacturers should take in consideration the fact that people might try to bend the handle in the opposite direction, and use a material a little less flexible, or a material that can handle more flexural stress. Conclusion: This type of failure is common in water bottle carrying handles, but if people use the object normally, and do not apply any flexural stress, and if manufacturer take into consideration the flexural stress that might be applied when used by customers, this type of failure can be easily avoided.
