In a 750-1,000 word paper, identify a population, a health issue affecting that population, and modifiable health behaviors related to the health issue. Select a theory or model to guide the development of an intervention for your topic. Include the following: Discuss the health issue you are addressing and describe the population it affects. Explain how this health issue affects this population and why this population is more at risk than others for this health issue. Identify and discuss three or more health behaviors that can be realistically be changed. Explain why these behaviors can be changed and how changing these health behaviors will prevent the health issue from happening. Identify a model or theory used in public health that can be applied to help inform an intervention for the health issue affecting your population. Discuss how this theory or model will help support behavior change for your intervention and explain why this theory or model is best for supporting behavior change. Based on your selected theory or model, propose an evidence-based intervention to address the health issue for your population. Explain how the intervention will assist in improving health outcomes for this population. You are required to cite to a minimum of three sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and public health content.
