This Assignment Dropbox is for your reflection questions from your Self-Guided Site Visit.  It is up to you to schedule and complete this site visit reflection, and you can visit a location that is out of the state of Maryland if you are home on Thanksgiving Break and would like to complete the visit. You only need to do one visit....but not to an assisted living facility, as we've already done that.

If you live on campus, and do not have transportation, may I suggest that you visit Bykota Senior Center which is on 611 Central Avenue, Towson MD 21204 and is a 10 minute walk from campus.   It is near Bosley Avenue, and they are very receptive to visits.  I have attached a copy of site visit reflection questions that you can use for this purpose. 

If you want to visit a senior center near your home, you may want to use these Senior Center questions for this purpose.

I have also attached a copy of a generic set of site visit questions for a skilled nursing home or CCRC community. 

It is highly recommended that your visit be to a long-term care facility other than an assisted living community so you are able to compare your Brightview Towson visit to a visit to another type of long term care provider or service.

HCMN 413: Services and Housing for the Long-Term Care Consumer Skilled Nursing/ CCRC Site Visit Questions Please use these Site Visit Questions if you choose to visit a Skilled Nursing Facility. Please make sure that you start your reflection paper with the name of the facility, address, and the name of the administrator. Make sure that you conduct your visit from the perspective of a family member that needs to look at a skilled nursing facility residence for a loved one. If you visit a continuing care retirement community (CCRC), please make sure that you answer question #3. If you do not visit a CCRC, you do not have to answer question #3. 1. Prior to your visit, please go to the Medicare Nursing Home compare website (https://www.medicare.gov/nursinghomecompare/search.html), locate the facility, and review the information posted for the building. A CCRC will have a health care unit, and you can review the rating for this part of the community from this website as well. How many beds are in the skilled nursing building/health care unit, and what types of rooms are available (are there single occupancy rooms, double occupancy rooms, any quads)?. Ask to see one of the rooms. Does the room look like the room in a hospital or nursing home – why or why not? What types of activities and rehabilitation services are provided by the organization for their residents? 2. How many administrators are in the building – is there one licensed administrator, or is there a licensed administrator as well as a licensed assistant administrator. What type of staffing is provided (number of individuals per resident, are they GNAs, is there a nurse that supervises the provision of services) and what is the facility’s star rating for the building. Is there a medical director onsite in the nursing home? Is there a social services department, and if so, how many licensed social workers are available? 3. FOR CCRC VISITS ONLY: What types of services are available in this retirement community? Do prospective residents have a “buy-in” or downpayment that they need to make to enter the community, and if so, what is this payment 4. Does the building have any residents needing dementia care? If so, how are those services provided – is there a specialty floor or area where they are cared for? Does the building have any residents that are not older adults (ex: younger individuals with disabilities)? Is there other specialty care offered by the nursing home (ventilators, traumatic brain injury, dialysis services, bariatric care, etc.)? 5. What type of food service is provided by the nursing home? Is it possible to get specialized meals such as diabetic meals, low salt, etc. Are kosher or halal meals available for residents on request? Are alternative meals provided for residents if they do not want to eat the food being served on a particular day? 6. What is the overall Star Rating for the facility, and what are its star ratings for its health inspections and quality measures? Please look at the latest health inspection report as well as the quality indicator report. Are there any interesting trends in these reports that helped you to better understand how care is provided in this building, and do you agree with the Star Ratings for the building? Please also ask if the nursing home has introduced any culture change initiatives in its services. 7. What did you learn about skilled nursing facilities or continuing care retirement communities after this visit? Would you refer a loved one to this building if they needed skilled nursing care – why or why not?
