Medicaid is the public health insurance program for low-income Americans. The
program has approximately 75 million beneficiaries countrywide representing one in every five
low-income individuals and families in the U.S. Almost all of the enrollees in Medicaid, have no
other viable options for affordable health insurance. However, although this population are
similar in their level of income, their demographic features are diverse. It covers parents,
children, pregnant women, older consumers, people with disabilities and other low-income adults
depending on the state. The program provides a variety of health services to this population
reducing personal medical spending. Those insured under Medicaid incur $921 less per year in
their out-of-pocket health costs (Clemans-Cope et al., 2013). This paper will examine Medicaid’s
goals, design, implementation and effectiveness as well as how it serves the professional/moral
obligations of advanced registered nurses in health promotion.
Income is a recognized cause of health disparity. A health disparity is a health difference
that is closely associated with socioeconomic or environmental disadvantage. Disparities can
exist along many lines such as racial/ethnic, gender, age, sexual orientation, disability and
socioeconomic status. All these variances determine a person’s ability to attain optimum health
(Healthy People 2020, 2016). The World Health Organization identifies income and social status
as one of the determinants of health stating that low social status and income level are linked to
poor health. The US national government is committed to reducing and if possible, eliminating
health inequalities. There are many national initiatives aimed at addressing this issue including
Healthy People 2020 and National Partnerships for Action to End Health Disparities among
others. These programs target to achieve access to comprehensive quality healthcare. Insurance
