How do ethical and moral considerations conflict with federally enacted laws that protect the rights of older adults?

According to the U.S. Constitution, Americans have the right to make decisions for themselves. An adult that is considered to have the decisional capacity has the unquestionable right to decide which form of treatment is right and the right to refuse any type of treatment even if refusal could lead to death. The conflict lies in the circumstance of when a person is unable to make a decision and one is made for them. For example, say you are in an accident and no next of kin or relative or spouse can be contacted. The doctors decide which is the best form of treatment because you are unable to make a decision due to circumstances. When a patient comes to after treatment has been completed and realizes that they will never be the same, this arises the conflict. I know that if I were in an accident and the decision was to have my leg removed to survive or to allow the injury to take my life, I would want the later. Life altering changes that are made by another's decisions raises the moral and ethical conflict of would a patient be comfortable living with a drastic change and what could this drastic change do to them mentally and in  the long run physically.
