Bowie State University Bowie, MD 20715 College of Professional Studies Department of Nursing Well Child Developmental Assessment Paper Guidelines Students will conduct a developmental assessment met in a home setting to identify factors influencing a child's development. WELL CHILD DEVELOPMENTAL ASSESSMENT PAPER (20%) Each student will conduct a pediatric developmental assessment in order to facilitate his/her learning of the multiple, predictable aspects of a child's growth and development. The student will also assess the child's home environment to identify some of the factors influencing the child's development. Following the visit, a written paper will be due which will include physiologic and psychosocial assessment data, goals for the child and family, interventions, and recommendations. I. PROCEDURE FOR THE VISIT Identify a well child (1 month – 10 years of age) either in a home or school environment. The child should not be a member of the student's immediate family. For home visits, you will be conducting a developmental and a home assessment for a child birth to 10 years of age. For school students, you may conduct a systematic assessment of their developmental status and identify environmental factors located in the school which are aimed at stimulating their development. Some suggested parameters to include: a) birth date, age, and gender b) growth parameters - use growth charts based on the ➢ For children < 2 years, use the Birth to 36 months 3rd-97th percentile forms and plot the following on the chart: o length for age o weight for age o head circumference for age o weight for length ➢ For children > 2 years, use the 2-20 years 3rd-97th percentile forms and plot the following on the chart: o stature for age o weight for age o BMI for age c) nutritional status d) development – remember the different aspects of development e) family ➢ Who lives in the home and what are their roles with the child? ➢ What influences do they have on the child's development? ➢ Is the child cared for outside of the home and what impact does that have on the child and his/her family? f) home environment or school assessment– ➢ include safety issues that may not be covered by this tool, i.e.: guns in home, helmets w/ bikes, harmful chemicals within reach, etc. 2) ANALYSIS OF DATA Bowie State University Bowie, MD 20715 College of Professional Studies Department of Nursing a) Interpret the child's growth percentiles. b) Describe and interpret child developmental findings. Select at least two developmental theorists and compare the child's development. c) Assess the child’s environment in the areas of cognitive and social emotional support, safety, nutrition, and list factors that facilitate or inhibit the child's growth and development. Or, if school based, describe environmental factors you have identified that are stimulating the child’s development d) Discuss problems to be addressed, nursing diagnoses, and needs. If there are no problems, discuss anticipatory guidance needs. 3) GOALS - for child and family 4) INTERVENTIONS OR RECOMMENDATIONS - to maintain and promote growth, development and health of the child. Include documented rationale. 5) REFERENCE LIST - Use APA format. Well Child Developmental Assessment
Alvin is an eight-year-old well boy who lives with both of his parents. The assessment of
Alvin was conducted at his home in the company of both of his parents and with the parents’
informed consent for the exercise. The parents of the subject provided the researcher with the
relevant information for the assessment and the assessment was a success. This report presents
the assessment results, data analysis, goals, and recommendations provided by the researcher
after the assessment.

Growth Chart
Nutritional Status
The following is a 24-hour meal plan for the child on the day of assessment:
Breakfast: 1 cup milk, ½ cup cereal, 1 banana
10 am snack: ½ cup of yoghurt
Lunch: 2 ounces steak, ½ cup green vegetables, ½ cup passion berry juice
Dinner: 4 ounces fish, ½ cup rice, ½ cup potato
Family Assessment
The child lives with both his parents and a live-in house assistant. The family also owns a
poodle puppy as a pet. Alvin spends most of the afternoons with puddle under the watch of the
house assistant. The role of the house assistant is to care for the child’s needs such as food and
clothing. The parents offer moral and mental support to the child. The parents also direct the

child in his home and class activities while offering parental support. The poodle is the child’s
playmate.
Development
The child’s developmental aspects were found to be in the normal ranges. First, the child
weighed 25 kg (1 month before assessment) and 129 cm (on the day of assessment). Secondly,
the child was able to play with age-appropriate toys such as computer games and even ride a
bicycle. He communicated efficiently and showed complex development of language.
Additionally, the child’s mood was well-regulated hence showing no evident signs of a mood
disorder or instability.
Pediatric Home Environment
Majority of home environment aspects show that the pediatric lives in a supportive and
safe environment. The parents are responsive to child needs and offer the child support in his
activities. Moreover, the child has access to age-appropriate toys and freedom to play in a safe
play room. The child is also offered the assistance of a care giver whenever dealing with difficult
tasks. The child also has his own room with clothing and bedding as required. The only issue
identified with the environment is that the pet’s vaccine schedule is not up to date. This presents
a hazard to the child since he can contact diseases from playing with the dog.

From the assessment of the child’s developmental findings, it can be said that the child is
effectively developing at his age and expectations attached to his age. This can be ascertained by
looking at Erikson’s stages of development. As an eight-year old, the child is in the industry vs.
inferiority stage of development which is typical for children between 6 and 12 years old. At this
stage, the child is keen to compare himself with peers and develops a sense of competition and
pride (Ricci & Kyle, 2009). This aspect of the child was noted from their tendency to show off
his toys as well as skills. Alvin was keen to explain how well he performs in school and how he
ranks top of his class. The main problem at this stage is usually the ability of a child to get along
with others. This goal was seemingly being accomplished by the child because he talked about
his friends and was evidently getting along with peers despite the pressure and competition.
Secondly, from Piaget’s theory of development, Alvin belongs in the concrete operational
stage and he effectively functions according to the demands of this stage. Children at this stage
are expected to engage in operational thought which allows them to conceptualize things in their
head (Ricci & Kyle, 2009). This was proved by the child’s science project which was not only
creative but also imaginative. Gauging from the child’s assessment, his developmental goals are
on track.
Child’s Home Environment
An analysis of the child’s home environment shows that he is well-supported
emotionally, socially, and cognitively. First, it was observed that the child lives with the two
parents who are married and hence accesses parental support in his activities. For instance, the
parents check his homework every evening and one of them assists with any challenging tasks.
Additionally, the parents also provide encouragement such as gifts when he performs well in
