Which within-subjects design should I use for my research about nail biting is a mental disorder or just a bad habit

It depends on what you are trying to prove, or if you are trying to prove anything. If you are just collecting data, I would recommend using a within-subjects design that tests all of the subjects using the same method.

For example, try putting something that tastes really bad onto all or the subjects nails.

The ones who taste it and don't bite, are probably habitual nail biters.

The ones that do taste it and continue to bite though it is unpleasant, probably have a mental condition or something similar. 

Also yo can bring in other factors such as how hard it was not to bite although it tasted bad.
