Description

Q1. The elimination of hazards is the first order of safety. The control of physical condition is basic and frequently difficult in a plant and it requires the best of scientific, engineering and management information for its accomplishment. The evaluation and control of mechanical and physical hazards encompass four engineering aspects: design, construction, operation and maintenance.

Choose a suitable process industry and conduct a HAZOP and risk analysis study for a particular section of the industry or the total plant. List out the various factors and identify the issues taken into consideration about the discrepancies in the design, operation, control and maintenance of the plant. Through this analysis also bring out various protection methodologies presently adopted by the industry and discuss its ability in meeting the safety norms and standards.( 2000 words )

Q2. Industrialization and related activities in the last few decades resulted in many issues with the use and handling of hydrocarbons and other components used in oil and gas process industry. Major accidents represent the ultimate, most catastrophic way in which a process industry project can go wrong. Accidents cause death, suffering & pollution of the environment and disruption of business. To minimize accidents it is highly important to assess and analyze various risk scenarios associated with the industry and projects.

Briefly explain about Quantitative Risk Assessment (QRA) techniques and mention its basic difference from other type of hazard analysis. Explain the objectives, methodologies and the procedural steps adopted in QRA. Choose any process industry and by using process flow diagram (PFD), briefly describe its salient features. Identify all possible hazards in the chosen section of the plant and tabulate process parameters (make suitable assumptions).List out minimum five failure scenarios and asses the risk quantitatively by assuming suitable failure frequencies. Hazard Identification and management in oil and Gas industry using HAZOP
Abstract
Hazard management within gas and oil industry serves a significant role in averting
accidents. Within gas and oil industry risk evaluation is done during the conceptual phase to the
conclusion of the plant’s life cycle. Accidents within gas and oil industry offer disastrous
consequence to the industry and influence a nation’s economy. Explosion, fire and toxic gas
discharge from the gas and oil industry kill large number of workers, destroy properties, and
impact the surrounding (Paul, 2015). A lot of accidents such as Seveso and Flexibrough resulted
in the death of many persons, damaged property, as well as affected the surrounding. Over the
previous four decades, risk management from the running of hazardous facilities has been
augmented. These main accidents highlight the significance of process safety in gas and oil
industry. Management of risks is employed by gas and oil industry to manage the risks within
their operation. This paper looks to study the use of HAZOP by gas and oil industry to detect and
manage the hazards.
Introduction
Safety within the design of gas and oil as well as offshore plans depends on diverse
design codes that are founded on the broad knowledge and experienced specialists and experts
within the industry. Hazard detection and management is basic in the safety management
systems. Threat or risk denotes anything that has the prospective to lead to harm, injury, ill wellbeing and damage to property. Plant safety is decided to a great level in design phase hence it is
ideal to detect the risks during the early design stage. Hazard detection and management is done
by gas and oil industries in diverse plant stages (Paul, 2015). There are various hazard evaluation

methods followed by industries that are either qualitative or quantitative. Some risk evaluation
methods engage numerical assessment to some level. Hazard recognition is the basic step in
assessment of risks. A lot of methods are adopted by industries to detect the risks. In case a risk
is not detected then the assessment of risks did not cover the entire hazards. The undetected
hazards might strike any moment which leads to loss and accident. As a result, it is important for
hazard detection to be done in a comprehensive way. HAZOP analysis is one of the methods of
hazard detection that may be used on various stage of project development. This paper looks to
analyses the use of HAZOP in identification and management hazards in gas and oil industry.
