For this assignment, you complete Milestone 2 of your Final Project. This milestone incorporates feedback from Week 1 on Milestone 1, but focuses more on the theory/model you have chosen to serve as a framework for your chronic disease or condition on a population level.

For this milestone, you explain your health theory/model in more detail. In addition, you create a visual representation of the health behavioral theory/model to help you to develop an argument for why this is the best fit for the intervention you are developing. Because presentations are visual-based exercises, it is important to be able to visually represent what the health behavioral theory/model will “look” like in the program being developed. For this exercise, you should be developing an original graphic. This means that you may use tools (e.g., software draw and shape tools) you find while doing research as the basis for creating your own graphic, but may not duplicate an image or graphic from a text or other source. The graphic you develop should be specific to your topic and should address the population you have selected for your Final Project.

To Prepare:

  • Review Part I of the Final Project Instructions provided in this week’s Resources.
  • Review the health behavioral theory or model you selected in Week 1 to address your chosen chronic disease or condition at a population level as well as the articles you researched for your Final Project.
  • Consider how your chosen health behavioral theory or model can be depicted visually to illustrate its application to your chosen chronic disease or condition and for the population you selected.

Assignment

Submit an original graphic representation of your chosen health behavioral theory or model as it relates to your chosen chronic disease or condition at a population level. If the health behavioral theory or model is illustrated in research, represent it in another way to ensure that your graphic representation is both original and relevant to your Final Project chronic disease or condition and population.

Address the following:

  • Identify your chosen health behavioral theory or model.
  • Identify your chosen chronic disease or condition and population.
  • Describe the graphic and its significance to the chronic disease or condition at a population level in more detail.
  • Be specific and indicate:
    • Key elements of the health behavioral theory or model and how they relate to your chosen chronic disease or condition at the population level including the significance of these elements and relationships between them
    • How this health behavioral theory or model applies to a population versus individual level
    • Include a key with necessary labels and terms described.

In the event that there is excess glucose in the body, a healthy condition known as diabetes mellitus develops. There is the other type of diabetes, which is termed as diabetes insipidus ; this condition is caused by low glucose level in the bloodstream. However, diabetes insipidus is less common. As the economic and social effect of diabetes progresses, there is also a direct increase in the variety of ways to augment the quality of healthcare services and reduce expenditure on health care amongst patients. Diabetes has turned out to be a stiff challenge in society, and various nations have stepped into the matter, to reduce its effects and different costs related to it. As a matter of fact, diabetes is one of the chronic diseases that has led to the death of many people and in the event that immediate action is not taken, its effects will be substantial in the general public. The condition is mostly associated with some of the lifestyles and other predisposing factors like age and obesity ("Management of diabetes at diabetes camps. American Diabetes Association," 1999). The research shows that the more you grow, the more you are likely to be diagnosed with type 2 diabetes. Consequently, those people who have obesity and do not engage in exercises are also expected to be diagnosed by this type of diabetes. The population that is mostly affected by this disease is older people; it primarily impacts the individuals that are aged between 30 years and above. Apart from the age category, the other group of people that are targeted in this case is obese individuals who rarely exercise. One of the facts about diabetes is that it can affect any person as long as he or she is leading an unhealthy lifestyle. There are those individuals who may not be sick, but they may wish to understand the principle of this model to help friends or relatives who have diabetes. The model chosen to serve as the framework in the management of diabetes is Health Belief Model, which is a behavior change in a health setting and was formulated to predict and explain some behaviors related to health, specifically those connected to the health services uptake. Social psychologist established the model back in the 1950s in the United States (Wdowik, Kendall, Harris, & Auld, 2001). It is the greatest and the best tool which is widely used to manage various diseases, including diabetes. Generally, the model is mostly concerned with the behavior change, whereby patients are required to adopt a self-care behavior, which is very important in the management of chronic diseases like cancer and diabetes.
