The intervention is a culturally adjusted Diabetes Self-Management Education and
Support (DSME/S) for African-Americans. Since African-Americans disproportionately suffer
from type two diabetes (T2DM) (Centers for Disease Control, 2017), the intervention aims to
reduce this burden by addressing challenges facing African-Americans in T2DM self-care. This
process will involve fine-tuning several aspects of the DSMES, such as point of access,
language, facilitators, and messaging. Specifically, the primary goal is to attain healthy
glycosylated hemoglobin (HbA1c) levels, which coincides with a reduction in diabetes
complications. However, other indicators of diabetes self-care efficacy such as body mass index
(BMI), blood pressure, and self-reported quality of life will be evaluated.
The planned intervention will take a six-month duration to conclude. The first month will
involve planning five of the subsequent months will compromise of execution and evaluation.
Planning will delineate the geographical scope, identify stakeholders (both facilitators and
participants), pinpointing specific target behaviors, and setting time-bound objectives as well as
