Medicare Documentation Guidelines as they relate to quality & cost in health care & the
NP role

Current Status
The United States health care system faces notable challenges that call for reform. There
are approximately 46 million Americans that have not been insured hence are faced with
increased premiums. Therefore, there is a need to address the shortfalls pertaining to quality and
efficiency that has led to increased costs and poor health outcomes (American Association of
Nurse Practitioners, 2013). The health care system has underutilized preventive care. This has
led to increased spending on complex and advanced diseases. Most of the people do not receive
the health care services that they require from the health services. Additionally, patients with
chronic disease have not received effective treatment. Some health care providers lack the
payment support and other tools necessary to enable effective communication and coordination
that would improve patient care (American Association of Nurse Practitioners, 2013).
There is increased variation in hospitals to inpatient lengths of stay, procedures, and
testing, and costs from one state to the other, from hospital to hospital in the same state. The
variations have no apparent impact on healthcare. Gaps such as limited evidence on the
effectiveness of treatments and procedures, and failure to detect and minimize errors exist in the
health care system (American Academy of Family Physicians, 2012). Reforms will address the
escalating costs and poor quality of care. Americans without health insurance coverage will
benefit as a result of these reforms. Furthermore, reforms shall improve access to affordable care
while keeping people healthy. This report reviews the health guidelines relating to quality and
cost of healthcare as it discusses the role of the Nursing practitioners towards health care
services.

Nurse Practitioners (NPs) and other advanced practice registered nurses (APRNs) are
important since they take care of patients and are more like their first point of contact n the
healthcare system (American Academy of Family Physicians, 2012). When they are left to
