Introduction
The work setting chosen for this paper is a telemetry unit with an initiative to

reduce readmission rates of elderly patients suffering from heart failure. The success of this
program is pivoted on the presence of interdisciplinary teamwork. Several functions within the
setting can be improved. One of them is the effective management of conflict. The second one is
achieving a streamlined communication pattern, and finally, the appreciation of diversity with
regards to culture, generation, and ways of thinking. The interdisciplinary teams are aimed at
improving clinical outcomes. Through the application of the aspects of compassion, advocacy,
resilience, and evidence-based practice, patients with heart failure can reduce their readmission
rates by thirty days.
Compassion
When compassion is combined with interdisciplinary teamwork, the collective
term for the same is compassionate, collaborative care. In healthcare, compassion forms the basis
of effective collaboration. Compassion can be described as recognition of and reaction to the
suffering of other individuals. When compassion is practiced in the absence of collaboration, the
result is usually uncoordinated healthcare. Also, when there is collaboration is without
compassion, the result may be correct in technical terms, but the emotional and psychological
