- An example of a common breach and cause: A common error when administering medications is administering the wrong dose of a medication to a client (McCuistion et. al. 2018; Cloete, 2015). According to Cloete (2015), nurses' often prepare medications around several distractions. Preparing medications with distractions such as noise and interruptions can lead to collecting an incorrect dose and administering it to a client, thus causing a medication error (Cloete, 2015).

- An example of prevention: Distraction is identified as a common cause of medication errors when administering medications to patients (McCuistion et. al., 2018; Cloete, 2015). In Cloete's (2015) review of how to reduce medications errors Cloete found that by developing a quiet zone, an area where nurses can collect medications without distractions, will decrease the chances of a nurse administering the wrong dose to their client.

A safe administration of medicines is important to the wellbeing of the patient. A mistake
done when offering medication may lead to serious consequences. For this reason, a safe cultural
environment should be embraced. This should begin by medical practitioners being vigilant
when preparing medications. Individuals responsible should carry their duties without
distractions. Additionally, before administering any form of medication, it is vital to assess if the
patient has any allergies. Nursing practitioners should always understand that they should
evaluate the patient before administering medication. For instance, they should assess crucial
organs, lab results, patient's data, and pain. Another essential factor that can encourage a safe
environment and culture is evading the frequent use of memory when providing treatment. When
nurses use their memory, there is a high likelihood of them making a mistake (Potter, Perry &
Stockert et al., 2016). Nurses and other medical practitioners should ensure that medication is not
expired. In case they have any query, they should consult with the relevant departments.
Therefore, by doing so, they will have created a safe culture and environment of medical
administration.
