If the applicant does not have the employer’s information: School official will try to obtain the
information from the employer. Some case the employer will not give that information over the phone,
then the student must obtain the information from the employer
Students that are between the ages 14-15 less than 18 hours a week during the school
year/Summertime 14-15 years old can work up 40 hours a week. 14-17 years old always need a work
permit
Students that are between the ages 16-17 can work any hours except school hours. During the school
year the student needs a work permit. However, the summer months the student is not require having a
work permit certificate.

Physical must be signed and stamp by physician
If the physical is not signed by the physician, Call the doctor’s office either to schedule an appointment
or confirmed that the student was seen within 12 months.
If the student plays sports/email or call the coach
Once the application is review
Work Permit Certificate is issue to the student
The student must sign the certificate in front of a school official
The student takes the application to the employer
Each time the student change employer’s the student needs to go through the work permit application,
the physical report is good for 12 months
Check to see if the job is vocation/ Regular employment
Make sure the student has a valid ID, (Driver License, State ID, or Birth Certificate)
Check Eschool to make sure that the student is enrolled in CMSD
Call the employer to ensure applicant is employed
Check for the employer’s correct address and tax ID number
Make sure the applicant has a physical/ If the physical is not stamp by a physician
Call and verified applicant physical (physician, coach or school nurse)

Who must apply for a Work Permit?
Any student who attends a Cleveland Metropolitan School ages 14-17 and is hired by an
employer.

Are students required to obtain a Work Permit for summer employment?
Student applicants who are age 14 or 15 are required to obtain a work permit in all instances.
Work permits are not required of 16 or 17 year old students unless requested by the employer
during the summer vacation. The parent/guardian’s written consent plus the minor’s proof of
age is to be kept on file by the employer.

Contained in this packet are three documents that must be completed and taken to the Work
Permit Office by the student applicant.

1. The Pledge of Employer from must be completed with a valid Tax ID Number and signed by the
Manager/Employer.
2. The student’s parent/guardian must sign the Application for Employment Certificate. The
school’s principal signature is not required. THE STUDENT MUST present a completed
application to the Work Permit Office at 1349 East 79th Street, Hearings and Appeals Office
❖ Proof of student’s applicant age (example: birth certificate, baptismal certificate, State ID, driver
license, temporary driver’s license)
❖ Physician’s Certificate The physical exam report must include an exam, the student’s height
and weight, blood pressure and pulse rate. The certificate must include the physician’s
signature and stamp. The student must have obtained this examination within the last 12
months.
As stated above, THE STUDENT MUST RETURN THESE DOCUMENTS IN PERSON TO SIGN THE WORK
PERMIT in the presence of the official issuing the certificate. THE PARENT/GUARDIAN DOES NOT
HAVE TO BE PRESENT NOR CAN THEY PRESENT these documents on the behalf of their child. The
Student MUST BE ENROLLED IN THE CLEVELAND METROPOLITIAN SCHOOL DISTRICT

SAIF - Expulsion / Involuntary Transfer

Determine whether to hold expulsion/transfer hearing. Hearing should only
be scheduled if it appears Due Process had been afforded and Hearing Officer
believes either Expulsion, Involuntary Transfer, or both would be warranted if
the allegations are confirmed at hearing.

If no hearing will be held, send Disposition to Principal and Network Leader
confirming the same, offer explanation, and provide suggestions for
addressing behavioral concerns. Place copy of email in file

additional evidence from school, Safety and Security, etc.

Schedule Expulsion Hearing - The Notice of Intended Expulsion is completed
and mailed via U.S. Postal Service and, if available, email. The written notice
contains the reasons for the intended expulsion and includes the location of
the incident, time and place of the hearing, the right to have a representative
of the student's choice at the hearing (the representative can serve as counsel
and need not be an attorney). The notice must be given 3-5 days in advance
of the expulsion hearing

A copy of the notice is e-mailed to the school administrator and the Network
Leader. The email should request that, even though notice has been mailed /
emailed, Principal provide copy of Notice to student or parent, if in contact.

Hearing and Disposition
Task
If requested by family, provide redacted evidentiary documentation to family
in advance of hearing
Complete evidentiary redactions, if needed
Convene Expulsion Hearing:
• Include referring building administrator on conference call
• Discuss the rule/offense that the student allegedly violated according
to the CMSD's Student Code of Conduct.
• Legal rights, procedures and possible consequences are explained.
• Inform student/parent or guardian of their right to hear the school's
report (SAIF, Serious Incident Report, CPO Offense/Incident Report,
Witness Statements, and Manifestation Determination Report for
students receiving special education services).
• The school referral is read and the supporting documentation is
reviewed.
• Offer student the opportunity to explain their alleged involvement.
• The parent/guardian is also given an opportunity to discuss the
incident and how it has impacted the student and the family.
• Confirm email address on file with family and document whether
family agrees to receive disposition electronically
Consider, on a case-by-case basis, appropriate factors, including, but not
limited to, a student’s age, health, disability status, special education status,
the perceived intentions of the student, mitigating circumstances, and
available behavioral supports as alternatives to expulsion. Follow-up with the
administrator may provide useful information on these areas.
Draft Disposition
If recommending expulsion, identify appropriate duration. If Level 4 and
recommended duration extends beyond academic year, determine whether
carry-over is appropriate.
If recommending involuntary transfer, identify appropriate receiving school
• utilize Division of Special Education, if student has disability
• may consult with family regarding placement preferences
Provide Family Notice of Expulsion, Abatement Letter, or confirmation of
closure via email and/or U.S. Mail
• If expulsion issued, provide list of public and private agencies to
improve attitudes and behaviors
• If expulsion issued, include information regarding re-enrollment
