N493 Community Health Nursing II
Course Description:
N493 Community Health Nursing II builds upon the foundation provided in N492 Community Health
Nursing I. From a theoretical and scientific framework, the concepts of epidemiology and disease
transmission, comprehensive assessment of risk factors and health problems, program planning and
intervention, environmental health, and collaboration with the interdisciplinary team are explored. The
practice experience component focuses upon developing and evaluating health promotion programs,
family assessment, community assessment, and community-based home care within the context of the
community. The nursing process is applied with the goal of promoting and preserving the health of
populations. It is anticipated that 70-100 practice experience hours will be required to complete this
health promotion project. **Please note, students who hold an RN license in WA are required to
complete a minimum of 80 practice experience hours in this course.
Program Learning Goals:
The goals of the RN-to-BSN program are to educate professional nurse generalists who emulate the
following characteristics and behaviors:
Professional Behavior/Ethics: Develop a personal values system and value-based behaviors that include
the capacity to make and act upon ethical judgments
Cultural Competence: Recognize the need for evidence-based knowledge and sensitivity to variables
such as age, gender, culture, health disparities, socioeconomic status, race, and spirituality
Research and Evidence-Based Practice: Use research findings and other evidence in designing,
implementing, and evaluating care that is multidimensional, high quality, cost-effective and leads to
improved patient outcomes
Leadership/Advocacy: Work within organizational and community arenas to promote high quality
patient care through organizational and systems leadership, quality improvement, and safety
Technology: Design and implement measures to modify risk factors and promote healthy lifestyles,
utilizing emerging advances in science and technology
Professional Standards:
The RN-BSN program is aligned to the AACN Essentials of Baccalaureate Nursing. This course aligns with
Essential 7: Clinical Prevention and Population Health.
Course Learning Objectives:
Apply a knowledge base of community health nursing and health teaching skills to the development an
educational project with a focus on illness prevention, health promotion and/or health maintenance of
individuals, families and groups (PLG 2, 4)
Demonstrate use of information technology in data gathering and analysis of a community or a specific
population group (PLG 5)
Analyze a community assessment to plan a health teaching project that meets the needs of communities
and populations (PLG 3, 4)
Create an educational project that can be utilized by public and community health nurses to improve the
health status and eliminate health disparities of individuals, families, communities and populations (PLG
2, 4)
Implement the planned teaching project to the selected target population(PLG 4)
Collaborate with community partners to provide education designed to improve population health(PLG
4)
Record and present evidence of the completion of each phase of the teaching project (PLG 3)

Course Structure and Assignments:
Module 1: Community Educational Project Site Selection and Letter of Permission
Assignment:
1. Select your site for the community educational project.
2. Complete and submit the Community Health Educational Project Permission Form.
3. Project Hour Log (estimated 1-2 hours)
Please submit your site selection in an APA formatted essay of 250 to 300 words in length. The essay
should explain why you selected the site and when and where your educational presentation will take
place. Please submit the essay, the permission form, and your N493 Project Hour Log. These may be
submitted in three files in the dropbox.
Module 2: Analysis of Community Assessment
Assignment:
1. Complete and submit the Community Health Nursing Project Hours Log (estimated 10-15 hours for
Module 2).
2. Discuss aspects of a health concern not being addressed despite the efforts of services and
partnerships involved and describe the ultimate outcome(s) or goal(s) from Healthy People 2020 relating
to that specific health concern.
Answering the following questions may assist in data interpretation:
• What similarities are apparent between the data that were gathered and the data that were
generated?
• What differences are apparent between the data that were gathered and the data that were
generated?
• What are the weakness and strengths of this community?
• In what areas is improvement needed in this community?
The assignment should be written in an APA-formatted essay. The essay should be between 1500 and
2000 words in length and include at least two scholarly sources other than provided materials. Please
submit your current, updated N493 Project Hour Log.
Module 3: Identify Specific Audience for Community Educational Project
Assignment:
Complete and submit the N493 Project Hours Log (10-15 hours for Module 3)
Create a presentation to illustrate and explain how the core determinants of health are impacting the
health of your target population utilizing the picture below:

The presentation should be between 5 and 7 minutes in length. Please use www.screencast-omatic.com to record your presentation in mp4 format and upload into the dropbox along with your
updated, current N493 Project Hours Log.
Module 4: Plan Community Educational Project Goals, Interventions and Outcomes
Assignment:
Complete and submit the N493 Project Hours Log (10-15 hours for Module 4).
Submit an essay of the developed action plan for a community educational project. The project plan
should include 2 (two) goals and objectives, an implementation plan for implementing these activities, a
budget, a justification for the project, a description of where and when the plan will be implemented,
and a description of the teaching materials which will be utilized during the execution of the project.
Recommend nursing actions/interventions to improve the health concern and achieve your stated goals
and objectives. Discuss potential public and private partnerships that could be formed to implement
your recommendations and create a timelines (i.e. 3 month, 6 month) for expected outcomes.
The assignment should be written in an APA-formatted essay. The essay should be between 1250 and
1500 words in length and include at least two scholarly sources other than provided materials.
Module 5: Create Teaching Materials for Community Educational Project
Assignment:
Complete and submit the Community Health Nursing Project Hours Log (18 hours required weekly).

Submit an 8 X 11.5 color (one page Word or pdf. document) infographic teaching tool for your chosen
community. Please also submit a reference page in APA format with at least two references you used as
sources for the information on the infographic. Submit your current N493 Project Hours Log.
Module 6: Community Educational Project Teaching Event
Assignment:
Complete and submit the N493 Project Hours Log (10-15 hours for Module 6).
Put your project to work! Hold the event at the designated and approved location. Take photos or
record a short video of the location (this may be a table you have set up, a booth location or a meeting
room) and if permissible include the audience; this will be the evidence that the event took place.
Create and submit a PowerPoint presentation illustrating and demonstrating your project in action. Be
sure to include the date(s), the location(s), a description of the experience, a brief list of people who
were involved and the amount of time spent for each event. The presentation should be at least 6 slides
in length.
Submit the PowerPoint presentation and your current N493 Project Hours Log.
Module 7: Community Educational Project Presentation
Assignment:
Create a PowerPoint of an overview of your project, then record the presentation (Screencast) of all
phases of the education project (this different from the one used at your event, if you created one for
that purpose) This will be presented as if you are in front of a group of peers explaining how your
community education project was planned, developed, implemented, and how the outcomes were
achieved and then how you evaluated if your goals were met.
Please make certain to show your ID at the beginning of the recording for student verification.
The presentation should be 7 to 12 minutes in length. Please submit either a link to the presentation or
the recording in mp4 format to the dropbox.

Module 8: Community Educational Project Evaluation and Reflection\
Assignment:
Summary
Briefly summarize your project. Who was your target audience and what was your topic? How did you
determine what your topic should be? When and where did you present the community education
project? Who gave you permission to present at the location? How did you advertise your
presentation? What educational materials did you use or provide to the audience? How many were in
the audience?
Evaluation
Explain how you would evaluate whether the efforts to improve the health concern were effective.
Include in your explanation the tools you might use to do this evaluation. Critically appraise your
performance of the activity/project for this population. How did the population respond? What aspects
were you most satisfied/least satisfied with? What changes would you make?
Conclusion
For nurses, reflection is integral to higher-level thinking. You should be able to critically analyze
scenarios and possible outcomes as opposed to simply asking the right questions. Acute perception of
the entire situation will be obtained because you are open to dealing with the contradictions that may
challenge your initial bias. Reflect on how your perspective of the community’s health and the national,
state, and local efforts toward a healthier population has changed as a result of your fieldwork. What
impact did your project have on your target population?
Please submit a recorded presentation of between 7 and 9 minutes that is a reflection of project. What
went well? What would you change in the future? You may video yourself discussing the project or you
may create a PowerPoint presentation and record your voice to the presentation. The presentation
should include at least two scholarly sources other than provided materials. Please be sure to provide a
photo of you at the location where you completed your education project. You may submit your
Reference page and photos in a separate file in the dropbox.
Please also submit your final practice experience hours log with a total of between 70 and 103 hours.
