Schneider, H., & Lehmann, U. (2016). From community health workers to community health
systems: time to widen the horizon?. Health Systems & Reform, 2(2), 112-118.
The primary reason for selecting the article was to gain insights into why the authors
believe that a shift from community health workers-based interventions to community health
systems is necessary. The other reason is to explore the significance of the article in depicting
how community health is tied to the wellbeing of community members. Moreover, the article
was selected to gain insights into the perspectives of the authors concerning the future of the
community, the achievements that it has had, and the health challenges that it seeks to address.
In summary of the journal, the authors recognize the vital role that community health and
community health workers have become integral parts of maternal and child health. Thus, the
article explores how community health has supported national health initiatives, especially those
aimed at delivering primary care to expectant mothers, children, the elderly, and HIV/AIDS
programs in low-income and middle-income states (Schneider& Lehmann, 2016). The other
main point that the article covers is the systems perspective on community health and community
health workers. Under this point, the article includes how a community health system will
prompt the shift from conventional challenges that have faced community health workers. The
authors also mention how the systems will facilitate the integration of community health
programs in primary care while being simultaneously embedded and supported by the
communities. Similarly, the authors believe that the emerging system perspective will propel
community health towards meeting its goal of achieving universal health coverage (Schneider&
Lehmann, 2016). The other main point covered in the article is what constitutes a community
health system. Under this point, the authors outline some of the over-looked actors in community
health and the critical features of the system. Schneider & Lehmann (2016) argue that
community health is defined by a set of local actors, relationships, and processes designed to
support health in communities outside the formal health structures, but within their scope. The
final important point addressed in the article is the implications of a widened focus for
community health strengthening. This point includes the influence of a widened community
health approach design in promoting community health.
As such, the most important thing that I have learned from the article is that the
community creates an appropriate platform for achieving the millennial development goals
(MDG) and attaining universal health coverage (UHC). For example, implementing community
health systems aim at providing a holistic approach for achieving UHC because it increases the
capacity of the community to act on health issues. Similarly, it seeks to gain the support of
volunteers in promoting community health; hence gearing towards UHC by eliminating barriers
in accessing health care programs. Thus, the community health system is a definable and unique
sphere that has presented an appropriate platform and roadmap for attaining sustainable health
goals such as universal health coverage and millennial development goals (Schneider&
Lehmann, 2016).
Besides what I have learned from the article, the primary benefit that I believe it brings to
the class is that it demonstrates how community health can be integrated into primary care to
facilitate the attainment of global health initiatives and the UHC. The other benefit is that it
presents that future outlook of community health, which seeks to help individuals in
understanding how to pursue future health initiatives. Moreover, the article is essential to the

class because it demonstrates how the implementation of community health systems will increase
the effectiveness of community health in bringing the greatest health benefits to the highest
number of individuals in need.

Abstract—Community health workers (CHWs) have reemerged as
significant cadres in low- and middle-income countries and are now
seen as an integral part of achieving the goal of universal health
coverage (UHC). In international guidance and support, the
emphasis is increasingly shifting from a focus on the outcomes of
CHW-based interventions to the systems requirements for
implementing and sustaining CHW programs at scale. A major
challenge is that CHW programs interface with both the formal
health system (requiring integration) and community systems
(requiring embedding) in context-specific and complex ways.
Collectively, these elements and relationships can be seen as
constituting a unique sub-system of the overall health system,
referred to by some as the community health system. The
community health system is key to the performance of CHW
programs, and we argue for a more holistic focus on this system in
policy and practice. We further propose a definition and spell out
the main actors and attributes of the community health system and
conclude that in international debates on UHC, much can be gained
from recognizing the community health system as a definable
sphere in its own right.
