Using Gordon’s Functional Health Patterns framework: assess the health risks in your community.
In your community assessment paper, include the following:
Introduction:
- Identify resources in your community that would enable you to complete a community assessment and submit a summary of your findings (1–2 paragraphs).
- Refer to Table 8-3: Examples of Community Strengths and Concerns, and assess the strengths and concerns of your community (2–3 paragraphs).
- Identify potential barriers to implementing community health plans in your community and brainstorm ways of addressing these barriers (2–3 paragraphs).

Community Assessment
Through personal assessment on my community, I identified an area of focus concerning
health risks whereby I came to realize that overweight and obesity is a health-related factor that
still affects the majority of individuals within the community or society (Bleich et al, 2018). In
most cases, obesity comes as a result of excess fats within the body. It is measured through the
use of Body mass index (BMI).
In carrying out my community assessment, some of the significant resources within the
community that assist in the completion of the evaluation and which enhances particular needs
for the individuals within the community are the people themselves. The assessment is to be
done on individuals to determine how obesity and overweight is a health-related issue within the
