In the United States, the population is made up of various immigrants who over the years have been forced to adapt to new lifestyles which they were traditionally not accustomed to. At their advanced ages, the immigrants, as is the case of Japanese immigrants, are left without anyone to take care of them, subsequently leaving them at the mercy of institutions such as healthcare day care centers. This discussion therefore,attempts to show how a bill proposed in the national Congress can be of benefit when it comes to addressing the needs of special interest groups such as the elderly in terms of quality healthcare service provision. As a control measure, the bill selected for this discussion had to be engaged in activities similar to those documented in the Japanese American Service Committee (JACS ) site. 3 H.R.652-Comprehensive care for seniors Act INTRODUCTION As of 1987, at least 12% of the American population consisted of the elderly. This percentage corresponded to approximately 30 million people in the then American population (Institute of Medicine, 1990). Over the years, this number has generally increased due to several factors. For instance, improvements in the fields of healthcare have definitely contributed to improved health among such individuals thus translating to an increase in the aged population. To support this statement, a study conducted on the American population revealed that in 2018, the total number of individuals aged 65 years and over was 52 million (citation?). Further, this number has been projected to increase two-fold in the next four decades to roughly 95 million elderly individuals in the country. Among other things, this population has unique characteristics such as the fact that it racially and ethnically diverse as compared to the elderly population in the 20th century.(citation?) With the growing population however, there is an impending challenge of caring for the elderly in daycares throughout the country. The United States, like any other western culture is defined by its own unique practices. Compared to Asian cultures where families stay intact, most families in the United States disintegrate at some point thereby leaving the elderly by themselves. Citation? Factors such as migration to urban regions have been attributed to be the leading reasons behind which such families tend to disintegrate.Citation? In many occasions, the elderly are usually left by themselves while the younger generation seeks out employment and other activities in order to sustain themselves as well as pursue their goals in life. Therefore, health complications associated with old age eventually forces such individuals to resort to daycares where they are able to get medical attention and care. This reliance on such centers has therefore resulted in day care centers in the United States being among the costliest health care systems globally (Eskildsen & Price, 2009). For day care targeting the elderly from minority communities in the general US population, the challenge is even more daunting. Some challenges that commonly face such as insufficient qualified personnel to help in running the day care centers as well as unavailability of sufficient funds 4 H.R.652-Comprehensive care for seniors Act to manage the homes effectively. Therefore, some institutions may end up shutting down their operations or selling out the premises to private investors. A common case in point is the Keiro Extended Care Center that was being run by the JASC but was eventually sold out to private companies due to the reasons outlined above. CITATIONS! Comprehensive Care for Seniors Act of 2019 The Comprehensive Care for Seniors Act of 2019 is a bill that advocates for the Centers for Medicare & Medicaid Services (CMS) to finalize a proposed rule regarding Programs of All-Inclusive Care for the elderly by April 1, 2019. The Program of All-Inclusive Care for the Elderly is a program encompassed under Medicare and Medicaid that is aimed at providing in-home and community services for certain individuals as an alternative to the commonly used nursing home (H.R.652, 2019). This program was developed with the thought of how expensive day care centers were to the elderly in the population. The proposed rule updates and revises application, enforcement, and other administrative requirements. The proposed rule was first published in August 2016 and its final action was expected to be conducted in August 2019. Once implemented, this rule would ensure that communities such as the elderly Japanese community in the American population, are able to receive appropriate health services in their advanced ages. History The bill was first brought before the House of Representatives on 17th January 2019. On the same day, introductory remarks were made by the sponsors. It was also referred to the Committee of Ways and Means as well as the Committee of Energy and Commerce for a period that was determined by the speaker of the House Nancy Pelosi. On the 25th of January 2019, the bill was referred to the Subcommittee on Health by the Committee of Energy and Commerce. Finally, on 1st of March 2019, it 5 H.R.652-Comprehensive care for seniors Act was again referred to the Subcommittee on Health by the Committee of Ways and Means (H.R 652, 2019). Sponsors The Comprehensive Care for Seniors Act, 2019 is sponsored by Representative Earl Blumenauer, a member of the Committee of Ways and Means as well as the Committee of Energy and Commerce. The bill has an additional 16 cosponsors (H.R. 652, 2019) . As a legislator, Earl has been involved in various campaigns mainly aimed at advocating for healthcare reforms. For instance, in 2009, he sponsored the America’s Affordable Health Choices Act of 200?, which was aimed at changing the then existing procedures to compel the Medicare to pay for end-of-life services. Other bills include the Bunning-Bereuter-Blumenauer Flood Insurance Reform Act of 2004 and the Senator Paul Simon Water for the Poor Act, 2005. The essential reason of sponsoring the bill is to ensure that the elderly in the society are able to access quality healthcare in an affordable and quick manner by providing the CMS with the authority of giving the final rule in the PACE programs. CITATIONS! Changes to Legislation Currently, Programs of All-Inclusive Care for the elderly (PACE) is known to offer quality healthcare services to individuals aged 55 years and older.CITATION The services are available to people who qualify to visit day care centers but instead chose to continue staying at home. One primary benefit of this service is that it has reduced the existing pressure placed on day care centers by reducing the number of occupants of such facilities. Currently, over 40, 000 elderly people throughout the country are being attended to by the PACE programs. Citation? The primary objective of PACE is to offer its services to deserving individuals due to the fact that the programs increase the quality of life of such individuals at a significantly lower cost to both the public as well as private payers. These workers are typically available for support throughout the year. In spite of these efforts, PACE programs still operate under outdated regulations designed in 2006. These laws therefore inhibit the effective working of the programs in the sense that there is little or no flexibility in the regulatory rules being used. The proposed legislation therefore aims at providing CMS with the authority of making decisions in the PACE final rule ("Cosponsor the Comprehensive Care for Seniors Act of 2019: Urge CMS to Release PACE Final Rule," 2019). This change would thereafter bring about flexibility in the PACE, and eventually make their services even more effective when it comes to offering healthcare services to the elderly in the society. The services offered include providing clinical treatment to the elderly suffering from various chronic ailments as well as providing the elderly with social support. JASC Site The Japanese community, just like any other Asian community in the United States is prominent due to several reasons. Primarily, their popularity arises from the fact that the Japanese immigrated into the country in large numbers due to a directive issued by the government of the US that removed barriers that prevented people from Japan and Asia in general to migrate to the country. CITATION? in the workplace by the American government to the citizens of the country and those of other Asian countries as well. The immigration into the USA was accompanied by several challenges. To begin with, unlike in Japan where the unity of the family is emphasized, the American society is more based on individualism, frequent mobility of family members in and the cutting of filial ties frequently. Therefore, these changes soon caught up with the Japanese society and over time, it resulted in the elderly among the Japanese to be neglected. To cater for their health needs, the JACS came up with an initiative of setting up an adult day care facility to cater for the elderly primarily because the available day care centers in Chicago could not meet the cultural requirements of the Japanese population. 7 H.R.652-Comprehensive care for seniors Act The implementation of this bill would therefore go a long way in supporting the efforts of facilities such as the mentioned Keiro Extended Care center facility. This is because the management of the facility would be under the CMS and therefore regulations being put in place would be flexible. Furthermore, the bill would play a crucial role in ensuring that facilities such as Keiro Extended Care center would benefit from programs being rolled out by the PACE staff and management. As for the time of developing this discussion, there was no indication whatsoever from the JASC site regarding their knowledge of the proposed bill. However, by virtue of the fact that the bill can be found on freeto-access sites, then it is safe to assume that the JASC is aware of its existence. A change in Comprehensive Care for Seniors Act I would make is the fact that minority elderly should get proficient care. Given the opportunity, the main aspect that I would change would be to impose the bill not only to the PACE program but to every other institution nationally. This modification would make it possible for the bill to benefit other day care centers whose services target minority groups in the country. Conclusion In conclusion, the Comprehensive Care for Seniors Act of 2019 is an important bill which when passed, will significantly help in improving the quality of life of elders. In the present age, factors such as improved healthcare services as well as proper nutrition have significantly increased the life expectancy of the society. This statement is based on the fact that statistics indicate that the number of the elderly in the population is set to double in the next half-century.
