6. You work in a urology practice, and a 51-year-old married patient, Mr. Ratner, undergoes a vasectomy because he does not wish to have any more children. He Confides to the Physician That he's concerned his mistress may become pregnant. He has the procedure performed while his wife is out of town, and he requests that information about the vasectomy not be disclosed to her under any circumstances. Thinking about your role in protecting the integrity of the medical record, address the following questions in your response:

Does Mrs. Ratner have the right to know that her husband has had a vasectomy? Does Mr. Ratner's physician or this staff have a right or obligation to tell her? If Mrs Ratner is also a patient in the practice, does that change your answer?

1. Describe the advances and disadvantages of an EHR.
The transition from paper-based record keeping to electronic approach can be attributed to
the following advantages of EHR:
First, EHR has led to improved quality of care through the reductions of errors and
misinterpretations that were a characteristic of paper-based record keeping. Secondly, the use
of EHR improves convenience and efficiency because the users can access the patient’s
health record just with a few clicks on the keyboard. Third, the use of EHR saves a lot of
space that could be used up by the bulky documents if papers were used instead. (Menachemi
& Collum, 2011)
On the other hand, the following drawbacks of EHR compromise its applicability.
First, the risk of a security breach and access to confidential information. Secondly, patients
may misinterpret information in the EHR, leading to unnecessary panic and alarms. Lastly, it
can be a costly measure to implement and maintain. (Menachemi & Collum, 2011)
2. Explain the difference between EHR, EMR, and Practice management software.
The focus of a practice management software the administrative and office work,
while EHR software is essential for documenting a patient’s medical information. Secondly,
dissimilar to practice management software, the US government promotes and incentivizes
the use of EHRs to increase the overall health care level. Lastly, front desk attendants and
office managers are common with the use of practice management software, whereas the
users of EHRs are normally the healthcare providers. (Menachemi & Collum, 2011)
On the other hand, EMR and EHR have a wide distinction between them. While
EHRs focus on a narrower perspective regarding a patient’s health record, ERMs cover

comprehensive data regarding the patient’s overall health. Also, compared to EHR, ERMs are
primarily used by health providers for treatment and diagnosis. They are also applicable to
sharing a patient's information with authorized providers staff members across different
organizations. Lastly, contrary to ERMs, EHRs authorize the movement of a patient's medical
information across different persons and units. (Menachemi & Collum, 2011)
3. Describe the basic function of EHR and explain clinical and administrative tools
found in an EHR system.
The basic function of EHR is to store a patient’s data in the electronic form for ease of
access and retrieval. However, there have been several definitions and suggestions regarding
the essential functions of EHRs. For instance, the American Academy of Family Physicians
noted that health care projects like Health Level 7 proposed the following possible basic
services of EHRs (Aafp.org, 2019):
o Identify and maintain a patient record
o Manage patient demographics
o Manage problem, medication lists, and patient history
o Manage clinical documents and notes
o Capture external clinical documents
o Present care plans, guidelines, and protocols
o Manage guidelines, protocols and patient-specific care plans
o Generate and record patient-specific instructions
The administrative tools within an EHR system include computerized tools like the
scheduling systems that help improve the efficiency of hospitals and clinics and their ability
to provide services to the patients promptly (Aafp.org, 2019). On the other hand, the system

contains clinically objective data and surveillance tools that can be used to tailor clinical
practices to patients based on their healthcare needs.
