- Read and be guided by any instructional terms in the Tabular List.
- Assign codes to the highest level of specificity.
- Code the diagnosis until all elements are completely identified.

The diagnostic process contains a physician’s statement, which contains the medical
condition from which the patient is suffering from. It also tells descriptions of additional
conditions and the symptoms from which the patient may be exhibiting. This is important as

index follows with the coder required to drill out the correct code based on the patient’s
