  3. Discuss how the results of previous research demonstrate support for the quality improvement initiative and its projected outcomes. Include a minimum of three peer-reviewed sources published within the last 5 years, not included in the course materials or textbook, that establish evidence in support of the quality improvement proposed.
  4. Discuss steps necessary to implement the quality improvement initiative. Provide evidence and rationale to support your answer.
  5. Explain how the quality improvement initiative will be evaluated to determine whether there was improvement.
  6. Support your explanation by identifying the variables, hypothesis test, and statistical test that you would need to prove that the quality improvement initiative succeeded.

Medication Reconciliation for the Elderly at Home
Medication is an essential aspect of the healthcare industry. The elderly, due to the
existence of several illnesses, have to take multiple drugs at the same time, which can be
confusing with time. Adverse effects have been reported among older adults as they forget to
take some medication or overuse others, leading to medical issues (Rappaport et al., 2017).
Medication reconciliation is an important aspect of extending care to the elderly, especially after
discharge from the hospital. Use of interventions to promote proper use of medication among the
elderly would help in improving their health outcomes (Spalla & Castilho, 2016). Indeed, having
an accurate list of all medication, their description, and how they should be taken can help many
older adults deal with the polypharmacy. While those in nursing homes have help from the
nurses in keeping up with the medication, older adults at home have a hard time adhering and
following the medication given (Spalla & Castilho, 2016). Therefore, having an initiative that
helps with medical reconciliation for older adults in homes would help in improving patient
outcomes and medication adherence.
Significance of Quality Improvement
Older adults have to deal with multiple illnesses that occur due to the many changes in
their immune system with age. Since medication is the most applied medical intervention, older
adults are in various drugs at one time. Missing specific medication is the leading cause of many
emergency department visits for older adults (Rappaport et al., 2017). Some of the patients report
that they forgot to take the medication or thought they had. These issues have led to adverse
