Health History
The patient identified is an African American male aged sixty-eight years old. He was
born in the year 1951 in September. He is married and has four people who are dependent on
him, including his wife and his two grandchildren. He has a degree in business management and
has been a loyal employee for Entry factories since the age of twenty-nine. He is, however,
retired. The patient has been diagnosed with diabetes and high blood pressure. Medications that
were administered to him include channel blockers to fight his severe high blood pressure
conditions and glimepiride for diabetes. The patient has, however, been fighting the side effects
of these medications, such as itchy skin. It is still recommended that he continues taking his
medication if he is to see results.
When it comes to the past medical conditions of the patient, he experienced mild asthma
during his adolescent stage. When he was the age of twenty-eight, he had an accident that
paralyzed him for six months. The patient has had immunizations for the following diseases
overtime; tetanus, hepatitis B, Rubella, Polio, and Influenza. He has had no recent travels and has
resulted in staying at home due to his medical condition. His father had diabetes type 2 at the age
of fifty. He experiences mild fever, fatigue, and sweaty hands. His skin and hair have retained
their regular coloring. However, his nails have begun showing signs of shedding and turning
yellow. According to him, his health conditions have not affected his head, eyes, ears, and
mouth. He, however, has a stiff neck from time to time. He experiences frequent colds and
shortness of breath. This sometimes results in chest pains. His breast condition is okay as he has
not experienced any lumps, nor has he undergone any previous surgeries. The patient
experiences a lack of appetite from time to time and instant cravings for some food types (Estes
et al, 2013).

He has no genitourinary conditions. The patient is sexually active and has encountered
one partner in the last three months. He has no prostate and scrotal problems but experiences
problems when it comes to sexual satisfaction. His final prostate examination and results showed
that he was in good condition. However, this was more or less ten years ago as the patient is not
sure when he underwent these procedures and tests. He experiences muscle pain and fatigue and
has had memory loss and problems with balance. He exercises daily, brushes his teeth twice
every day, and has had screening examinations for blood pressure. His nutritional pattern is as
recommended by the doctor and includes a daily intake of supplementary vitamins. He follows a
strict sleeping pattern of eight hours a day. He can do things on his own, which shows that his
functional ability is still okay. His habits include the consumption of tobacco and alcohol eve...
