For this project, students will design and develop a database that can be used to compile and report clinical data related to either quality measures or pharmaceutical products and utilization. The proposal is the initial step of this assignment.
Create a 500-750-word proposal outlining the following:
- Statement of the project problem and definition of terms.
- Identify your users (personnel/credentials): Who are they? What are their competencies?
- Discuss the information data users need to compile for the proposed system and the types of reports the proposed system should be able to generate and why.
- Based on your response to #3, articulate the project goals and objectives for the database design and how they affect the data model.
- Identify the steps you will take to meet the goals and objectives.
- Forecast any potential problems or barriers to meeting the project goals and objectives. Are there any specific assumptions that need to be made?
- Conclude with the value of the project with emphasis on feasibility, necessity, usefulness, and the benefit of the expected results.
This is an ongoing project and the feedback from the instructor at every stage will help you in improving your final project. Be sure to consult with your instructor throughout the course and incorporate suggestions and recommendations in your project.

Proposed Database which integrates Patient Demographics and Research for discharge
Status
Introduction
Patient demographics refer to the vital data that is analyzed in any or all medical
institutions. These demographics allow for statistical analysis which involve identifying a patient
and how one can categorize the patient. Patients can be categorized in various ways in the
medical domain and these include; gender, contact information, date of birth, ethnicity,
emergency contact information, blood type, and major diagnosis. Patient discharge status on the
other hand refers to a realistic conclusion of a healthcare encounter and it involves issuance of a
patient discharge status code which acts as an identification of the particular patient.
Problem Statement
This project was developed after a computer Science and a statistics team collaborated
with a reconstructive medical team at a prominent health facility to make easier the patient
demographic analysis as well as generation of patient discharge status codes. The project’s major
actor is the healthcare team from the health facility. The team wishes to devise a digital model
that works out well in planning, preparing, analyzing and recording patient demography as well
as assimilating the model to facilitate patients’ discharge by generating the patient discharge
status code (Date, 2019).
Digital Model- This is a contemporary representation of a statistical or mathematical formula in
the real world.

Patient discharge status - refers to a realistic conclusion of a healthcare encounter involving
issuance of a patient discharge status code.
Users
There are several targeted users of this digital model. The first crucial group is the
healthcare student’s administrators who need to employ this model in their research and for other
academic studies with their students. The students are a crucial part of a healthcare institution
and they study in the institutions around the medical facility. The other users of the model
include the ICT officers, inventory head, nursing department head, head of medicine, head of
equipment department and the security department head. The ICT head is the officer who is in
charge of all software that need to be used within the hospital and he or she ought to be the first
officer to learn about the model. The various heads of departments are in charge of all operations
within the healthcare institutions and hence the need to make them crucial users of the model
(Dimitrov, 2019).
Data collection
The ways that I would use to collect data necessary for my research conducting
interviews which involves going to the field of study and engaging a one on one talk with the a
respondent to obtain the needed information. I would also employ then use of questionnaires and
surveys. This involves coming up with questions that relate to the study and constructing an
exhaustive questionnaire that I would use to conduct a randomly sampled group of respondents
to respond to it. Another crucial data collection procedure would be going to the field of study

and literally observing the outcome. Other methods that I would use involve oral histories,
documents as well as records and focus groups.
Goals and Objectives of the Database Design
The main goals and objectives of my study would be the ones that are mentioned below:

c. Develop a secure data base design
d. Develop a database that can recover, administer and perform on data effectively.

Steps to meet the Goals and Objectives
In order to achieve the aspired long term and short-term goals of this study, I will need to

involved in the study. The second one would be to effectively use the data collection methods in
