Case management and care management
Case management is the combined process of assessing, facilitating, planning,
coordinating care, advocating and evaluating services and choices to achieve people’s general
health requirements. Due to the availability of resources and communication to promote the
safety of patients, ensure quality care and cost-efficiency. On the other hand, care management is
a comprehensive plan involving activities and services which help people suffering from
complex health matters and chronic illnesses to manage the condition.
Care management and case management are different, but several differences. In the care
management structure, the professionals involved are diverse, including nursing, social works,
gerontology, among others. In contrast, case management has only social workers, and nurses are
involved in giving primary care and mental health care counsellors may be there or not
(Miller, 2013). Care Management giving is mainly based on private facilities where most are
based on gaining profit. In contrast, Case Management is based primarily on governmental
programs, and organizations such as insurance bodies, mental health facilities in the community
and hospitals. Care Management main aim is to improve the quality of life, improve
communication among caregivers and enhance independence. Case Management, on the other
hand, aims to better the health status, reduce service overuse and to improve cost efficiency.
Case management concepts
This concept deals with the care of the patient and those related to them. It also contains
the complexities associated with the process, caregivers coordination, and the treatment process.
The concept is not yet clear on the issue of time. That is if the idea should include punctuality or
continuity. Case management surrounds the idea of patient assessment to know their needs, the

The main aim of caregiving is responding to improvement and providing continuous care to the
