Introduction
This community health assessment (CHA) involves the evaluation of population health in
the city of Philadelphia. The evaluation is critical to helps establish a particular health concern
for the given population. This well-conducted health assessment enhances the generation of data
on particular health concerns. Various methods are utilized to collect data on the community's
demographic, environment, social, and health factors. With a population of more than 1.59
million, Philadelphia is ranked the largest city in Pennsylvania and the 6th largest in the US. A
community health assessment highlights public health challenges and assets in a community.
This CHA for Philadelphia outlines health behaviors, environmental and social determinants of
health, health conditions, and healthcare conditions in the community. The CHA shows a leading
cause of death in the community is heart disease, among other chronic diseases. There is a high
level of adult cigarette smoking than any other major city in the country.
Defining the Community
Description of the Population
Philadelphia has an average population estimate of 1.59M in 2019, rising from 1.53M in
2010 to a 4.34% increase (US Census Bureau, 2019, April 04). This makes the community the
largest city in Pennsylvania and the 6th largest city in the US. On average, there is a population
density of 11854 people per square mile. The population is distributed by age as 6.8% below 5
years, 22% below 18 years, and 13.2% above 65 years. The population of the community has
continually grown each year since 2006 (PEW. (2019, April 11). Young adults and immigrants
influence population growth.
Races

The main racial groups in Philadelphia are Black, Hispanic, and white. The population
distribution by race is black 42.1%, non-Hispanic white 34.6 %( but 41.2% white), Hispanic
(Latino) 14.5%, Asian 7.2%, American Indian and 0.4% (US Census Bureau, 2019, April 04).
The migration of the black from the south to Philadelphia in the 20th century significantly
augmented its population. This makes Philadelphia one of the most important communities for
blacks. Chinese immigrants are also a significant portion of the foreign-born population in
Philadelphia, significantly the city (PEW, 2019, April 11).

Boundaries in the Group
Adults in Philadelphia have significant boundaries, particularly in the household. In this
community, the adults are either in both extremes for high incomes and high poverty. While the
community has the highest level of poverty among US 10 largest cities, the major concern is
income inequality. 25% of the population in Philadelphia lives in a poor neighborhood such as
Fairhill, while others live in rich neighborhoods like the Centre City. The majority of the poor
groups are black, although white poverty is also significant.

General Characteristic that Separates the group from others
Compared to other communities in Pennsylvania and the US, Philadelphia has a high
level of crime and drug abuse. Homicide and opioid-related deaths are a significant characteristic
of the Philadelphia community (PEW, 2019, April 11). The foreign-based population is 13.9%
compared to 13.5% for the US.
Education levels/ birth or death rates, age of death, insured or uninsured
The majority (83.9%) of people in Philadelphia have a high school diploma or more.
28.6% for individuals older than 25 years in Philadelphia have a Bachelor's or higher US
(Census Bureau, 2019, April 04). The majority of the people have health insurance, but 10.13%
below the age of 65 are without health insurance (Census Bureau, 2019, April 04). The number
of people with health insurance in Philadelphia grew by a third from 2008 to 2019 (PEW, 2019,
April 11). Below the age of 75 years, the year of potential life lost has decreased from 12200 (in
every 100000) in 2000 to 9000 per year in 2018 (Washington, 2017). However, for the African
Americans, the year of potential life lost are greater 11200 compared to 7609 non-Hispanic white
(Washington, 2017).
