Introduction
The type of assessment section will provide the kind of community health assessment tool that I
would use in Johnson County. The assessment will be responsible for giving Johnston County
comprehensive information regarding its community's current health status and other key issues
and needs. Additionally, this information will be used by Johnston County to develop a
community health improvement plan by justifying where and how the resources available will be
allocated to meet the community's needs.
Type of Assessment
The type of community health assessment tool used for Johnston County is the Health Needs
Assessment tool (HNA), which is essential to meet all the health priories within the county. The
tool will be used to commission and serve the overall plan. The device will also be used as a
systematic approach to identifying unmet health and the healthcare needs of the Johnston county
population. It will also be used to make changes that are required to meet unmet needs. For the
tool to be fully functional, it has to include the following key components and requirements:
The Assessment team and engaging community partners will form the team that will be
responsible for providing data to be used in Johnson County. They include community leaders,
vulnerable population, the health and non-health professions serving in the community as well as
the community as a whole that has unmet needs. Therefore, through the synthesis of the
secondary and the primary data, the health needs significant will be able to be determined.
Collection, Analyzation, and presentation of data: After a careful study of the community's data,
