The conflict between autonomy and beneficence is a common thing in healthcare settings
(Martela & Riekki, 2018). It often presents an ethical dilemma that nurses must deal with
considerably and from a patient advocacy perspective. Conflict of patient autonomy and
beneficence occurs when a patient's wishes are deemed risky for their health or unhelpful by the
healthcare team. Besides, such conflicts happen when a patient's decisions conflict with what
their family members need for them, as evidenced in this case. In this case, the nurse must make
evidence-based ethically and legally right decisions, respect patient autonomy. At the same time,
nurses must warrant that their decisions maintain and strengthen the trusting relationship with the
patient and their relatives.
If I were in this situation, I would tell the daughter, 'I know how you feel about your
mother and want the best for her. However, there are proved state, federal, and hospital policies
regarding deciding on this situation where your dearest mother's decision and your decision
conflicts. Usually, there is the Bill of Patient Rights, which protects patients and inform ethical
and legal decision-making. The Bill of Patient Rights comprises of patient autonomy rights and
other patient rights. Patient autonomy right provides that everyone obeys a patient's wishes as
long as they can competently make decisions regarding their health and treatment. Besides, it
states that no one, including the hospital and family, has the right to go against these wishes
unless they have permission from the patient. Doing otherwise is illegal and punishable at courts
of law for breach of patient rights. Based on patient autonomy rights, healthcare team members
and I are obliged to follow your mother's decision and wishes regarding her treatment because
she is competent according to our assessment. Presently, all I am only permitted to provide her
with adequate information to her about her care, treatment, and possible alternative treatments,
including their benefits and consequences, to help her with decision making. I can neither coax

her to decide otherwise or decide on her behalf. I know it is a hard time for you and your family.
Still, we have an effective and warm counseling team and other excellent resources to help
accept her wishes and adequately join us to support her decision. I am requesting you join us and
actively care for her, as you have always done. Your presence during care and active
participation is also therapy on its own and may result in desired outcomes'.
Patient autonomy is prioritized when it conflicts with beneficence according to the
Patient's Bill of Rights and various evidenced-based ethical decision-making models (Nelson,
2017). In this case, the patient is competent, meaning that she is legally permitted to decide on
her treatment whether the healthcare team and the family agree with it or not. As a nurse, patient
advocacy is my crucial role at every stage of care. I must always warrant that patients' wishes
and desires are obeyed, and adequately met irrespective of the dangers they post to their health as
long as they are competent. Also, I did this to help build and maintain a trusting relationship
between her, the healthcare team, and the family, which can motivate her to verbalize and give
crucial information that may promote her care.
Moreover, I comprehensively explained to the daughter the basis of granting obeying
mother's decision to help her understand that we care and understand both her concerns and that
of her mother and, at the same time, must adhere to evidenced-based set guidelines and policies.
