1. Action Plan: Recommendations for Change
There is a need to shift the attention from individual-based strategies to a population-based
scope. This can be achieved by strengthening collaboration between the public health units and
medical units and CHVs to ensure that traceability and medical follow-up can be done to ensure
management is not under duress. Also, this will ensure that hypertension is added to the
dimension of several other population health issues to give it the attention it needs in the
management process (Angell, De Cock, & Frieden, 2015). Also, improvement of surveillance
and the report method of hypertension will help characterize the health trends in general and
even among the subgroups of a community.
