The main task I undertook this week was to assess the needs of my healthcare setting and
the community served. I completed the task collaboratively with my preceptor who offered
guidance on the current state of the healthcare organization and most urgent needs in the
community. One of the core competencies demonstrated during this week’s practice was
knowledge and skills in identifying and responding to health disparities. A health disparity may
be defined as the burden of illness, mortality, or injury that one group of people experiences
relative to others (Ubri & Artiga, 2016). My practice this week typically looked at existing health
disparities among community groups and people in the hospital to determine the current health
and health care gaps. For instance, I identified that the older patients were experiencing high
rates of falls compared to other patients. I identified some factors associated to the disparity
including frailty and comorbidity. Therefore, my practicum especially when identifying potential
topics, included knowledge and understanding of health disparities and how they affect health
outcomes. Additionally, I demonstrated inter-professional collaboration by working with my
preceptor in the needs assessment and topic choices. This week was productive and I achieved
the goals of needs assessment and listing the potential capstone practicum project topics.
