Introduction
Hospital readmission is a significant challenge that almost every hospital across the
nation has to deal with. The reduction of hospital readmission rates is a major healthcare policy
goal since it provides an opportunity to improve healthcare quality, reduce healthcare costs, and
increase patient satisfaction. Although some cases of hospital readmissions may be necessary,
most of them can be prevented. It is important for hospitals to adopt change initiatives that can
help them to reduce the rate of readmissions. The proposed change initiative focuses on engaging
and educating patients before discharging them, identifying the causes of readmissions and
understanding patients' population at high risk, enhancing transitional care, and scheduling
follow-ups before discharge.
The issues around hospital readmissions and the current outcomes as a result of the issues
Readmission applies to patients who have been readmitted to the same hospital or another
healthcare facility within a period of 30 days after the first hospital stay. It is estimated that
among the elderly patients, one in five of them is readmitted after being discharged within 30
days. The costs associated with hospital readmission are significantly high, making it essential
for hospitals to address the issue by improving their processes. Medicare spends about $26
billion annually in hospital readmissions with about $17 billion being spent on readmissions that
can be avoided (Warchol, Monestime, Mayer, & Chien, 2019). Reducing preventable
readmissions rates is also an important factor that is considered in rating the quality of healthcare
provided by a hospital. Since there are known factors contributing to readmission, hospitals are
expected to utilize this knowledge to minimize readmissions. As a result of the high costs of high
readmission rates, especially in Medicaid-covered patients, the Affordable Care Act imposed

penalties on hospitals that have excessive readmission rates. These penalties affect the hospital’s
financial health.
The external and internal forces, contributing issues, and the people affected by hospital
readmissions
Both external and internal forces contribute the issue of hospital readmission. Among the
factors that contribute to preventable readmissions include lack of patient compliance with the
instructions given after discharge. Some patients don’t take the discharge instructions seriously
and ignore their follow-up appointments. Readmission is also contributed by several health
conditions that have a higher risk of leading to readmission. Some conditions have high
complication rates that increase the risk of readmission. Among these conditions include chronic
kidney disease, pneumonia, heart diseases, among others (Joo, & Liu, 2017). Inadequate
transition to care whereby a patient may not be aware of what to do next or signs of potential
issues also contribute to readmission. Discharging a patient too early before being adequately
stable also contribute to readmission. Some people are at high risk of being affected by
