Patient safety is a healthcare development that emphasizes the need for medical personnel
to reduce medical errors. The guidelines established to ensure patients' safety involve preventing,
reducing, reporting, and analyzing medical errors that may lead to adverse health repercussions
on patients. The number and prevalence of medical errors are especially high during the 1990s as
patients, and family members in many countries reported having been harmed or killed due to a
medical error. According to WHO, medical errors affect one in ten patients globally (World
Health Organization, 2017). Significant research has been conducted and theories established to
inform the improvement of patient safety worldwide. Findings from extensive research are used
to improve existing programs to reduce medical error while also improving the reporting system
and developing incentives to contribute to a work environment with minimized errors.
Multiple programs and policies have been set up by the government to ensure that
medical errors are effectively prevented and reduced. As part of the effort to ensure patients are
given proper avenues to voice their complaints, the Patient Safety and Quality Improvement Act
(PSQIA) was created. The Act provides health providers with a legal tool to report patient safety
concerns and relevant health information. To carry out the Act's mandates, the Patient Safety
