CASE OF DILEMMA IN NURSING

Introduction
Ethical dilemmas in nursing are very common. Nurses experience a variety of challenges
or dilemmas in their daily routine (Jie, 2015). Most of these ethical challenges or dilemmas are
more likely to happen in a situation where a nurse is presented with a patient at the stage of the
end of life (Jie, 2015). This study focuses on a case where nursing is faced with the task of taking
care of a patient at his end life.
The Attempt of Suicide by a Patient Case Study
Summary of the study’s facts
A 57 years old client named Mr. Green suffers from prostate cancer, and his cancer's
condition had reached the last stages. Mr. Green is under the care of nurses from the department
of oncology in a general hospital at QLD, Brisbane, Australia (Jie, 2015). Following Mr. Green's
disease history, it is revealed that he was instructed to go for a diagnosis in the last seven years,
but he chose to refuse the diagnosis of his cancer condition. However, rejecting the diagnosis,
Mr. Green resolved to the exploitation of other options. He went for other treatment options and
refused to follow the urologist for the past seven years (Jie, 2015). Mr. Green's current situation
has turned worse as the recent diagnosis that was carried on him indicated that his cancer
condition had spread by metastasis to his bones, it had also locally spread to his lymph nodes,
and there was an invasion of a primary tumor on his bladder obstructing the left kidney. Mr.
Green had been informed that he had only remained with 4 to 6 weeks to be alive (Jie, 2015).
This information made him resign on himself and developed thoughts of committing suicide. He
then confided to one nurse that he had plans of killing himself and directed the nurse not to share
the information (Jie, 2015).

The person faced with the challenge or dilemma is the nurse. The issue or problem that
presents an ethical challenge or dilemma to the nurse is whether the nurse who was confided
with the client's suicide information should share it with the team members or keep it a secret as
the patient requested. In this case, the nurse is the one who is also faced with ethical issues about
his or her nursing profession (Jie, 2015).
Ethical issues analysis using an ethical model of decision making
Moral awareness
Individuals or professionals are always required to be aware of the morals of their
professional ethics. The moral awareness component of decision making is the ability of a person
to appreciate and detect the aspects of ethics in making the decision (Park, 2012). The first step
in the ethical act is to be morally aware of the ethics governing the presented challenge. To
evaluate decisions that need to be made, everyone is required to keep ethics his or her
consumption. The application of moral awareness in decision making is evident in the case study
where the nurse exhibited the moral awareness of his or her professional ethics. In reference to
the morals, the nurse resolved not to keep the patient's secret and shared it with her team
members as their moral ethics dictated.
Moral judgment
The judgments used in situation evaluation, action courses evaluation, behavior, and
individuals' evaluation and has a moral judgment is referred to as moral judgement (Park, 2012).
This judgement is based on the philosophers' dispute, where some argue that moral judgment has
its basis in feeling or intuition that is emotionally connected. The moral judgment is applied in

2012). The behavior of ethics respects diversity, dignity, rights of groups, and individuals. The
