Introduction

Over the past decade, regulatory changes have created new opportunities for cross-sector collaboration around chronic disease prevention. The passage of the Affordable Care Act (ACA) in 2010 provided a legal foundation for widespread reform od the healthcare system around such goals as expanding health insurance converage, strengthening primary care access, incentivizing prevention, and increasing integration between healthcare and public health. Requirements for nonprofit hospitals to conduct periodic community health needs assessments, to engage public health partners in community health planning, and to report their contributions to their communities all contribute to an environment conducive to collaboration.

In this LearnScape, the student is functioning as an external consultant working with the Director of Marketing, at Bright Road Healthcare System, to find ways to improve the functionality and utility of the system’s patient health record (PHR) application. Not only does the PHR engage patients in their own wellness and a method of retaining loyal clients, can also be critical for the success of collaboration in integrated care delivery.

There is a growing focus on population health management. Population health management focuses on promoting wellness through an emphasis on prevention which in turn controls healthcare costs. If the population is doing a better job managing their health status, then they are less likely to access healthcare only when they experience an acute event. This should result in a reduction in the emergency room visits, which is perhaps one of the most expensive points of service in the U. S. healthcare delivery system. In order to make a comprehensive population health management initiative successful, it requires a high degree of collaboration between chief stakeholders (public health, hospitals, academic medical centers, insurance/managed care organizations, patient-centered medical homes, accountable care organizations, and, of course, the population). There is also a need to have adequate information systems to support such a comprehensive wellness promotion strategy. The personal health record (PHR) is one such system that is designed to permit patients to compile and maintain information about their health. In addition, clients retain control over how the health information is accessed, used, and disclosed to third parties. While the PHR is similar to an official medical record, they are not the same. The medical record contains information provided by healthcare personnel. The PHR and patient portals are examples of information systems that are designed to increase population health management, foster patient engagement, and promote repeat business to healthcare organizations and providers.

I believe if we can better inform patients on what is available to them and more importantly, teach them how to use the system, we will see more participation. The initial kick off and participation was pretty good, especially with our Generation X and Y patients. This group is more likely to use the internet and feel comfortable with the online security of their information. Patients were able to enter their medical history and current health conditions. And, they could share their medication lists and emergency contact information. All of this information is stored on a secure server that we host. JOAN RESPONSE 2 Why don't you start and I will chime in after you. AMIT RESPONSE 2 Okay, that sounds good. We had a good response initially when we launched the PHR. The patients who created profiles were from the demographic that we expected. But over time, these patients were not maintaining their profiles or keeping them up-to-date. Usage simply dropped off. We added health tips on a regular basis, but our feedback showed that they aren’t being read by our patients. JOAN RESPONSE 2 Yes, and we were hoping that we would have more participants from our older group of patients - those over 60 years of age. Because this is the group of patients that I primarily work with, I can see the benefit of them using the PHR system. AMIT RESPONSE 3 There are two main areas that we would like to see improved. First, we would like greater participation. Second, we'd like to initiate a campaign to help educate patients about how this service can help them, and how to use it to produce the best benefits for them. JOAN RESPONSE 3 I agree. And I would really like to see a way to get older patients actively involved in creating their PHRs and using them. This is a huge opportunity to help educate them about preventive care and alternative lifestyles that have an overall positive effect on their health. Personal Health Records are essential most, especially for the patient, to track and
monitor patient information. In the case scenario presented, two areas require improvement- the
need to increase patient engagement and educate patients on the benefits of PHR systems'
importance. This calls for a marketing strategy that draws the attention of consumers to increase
their engagement. It is essential to consider various technological, social, competition, and
regulatory factors in developing a marketing plan. On the technological front, the user interface
needs to be quite interactive. It should contain elements that are adding value to the user
experience. Amit noted that the organization does not have any problem with the current user
face.
Thus, the recommendation is to use accordions. These are vertically stacked lists of items
that utilize the show/hide functionality. This is more interactive as the users will not have to dig
deep into the site to get information. Additionally, there is a need to personalize messages
according to the user's conditions (Andrikopoulou et al., 2019). For instance, giving health tips
not on a broad scale but personally, which is also part of the PHR interactivity. Social factors
include the changes in the demographics and the lifestyle of individuals. There has been a
common trend where people of all ages are looking to living healthy lifestyles. This brings in the
other recommendation: to integrate the PHR with lifestyle and health applications (Fylan et al.,
2018). This will attract patients from all generations because there is a social trend of healthy
living and lifestyles. People are subscribing and installing health apps more than ever before. The
PHR should include evidence-based techniques towards behavioral change to enhance a healthy
lifestyle. This is an advantage over the health apps, which do not necessarily provide evidencebased information. Thus, the integration will benefit both the hospital, the healthcare app, and the
patients.
