Description

Seeking a Cause: PICO Question

Are adults in a inpatient setting (Population), who have prophylactic antihistamines pre- blood transfusions (Intervention) compared with those without prophylactic antihistamines at risk for/of blood transfusion reactions (Outcome) over the first 15 minutes of the transfusion (specific Time)? Is this best practice?

The purpose of this assignment is to write an evidence-based practice paper for a clinical problem identified in the student’s clinical practice setting.

The EBP paper will be developed from the PICO question addressing the clinical problem.

An in-depth review of the literature using primary sources will be completed.

Once, you have the answer to your problem, you will need to plan how you will address the clinical problem, which includes the conceptual framework (the theory you wish to apply), objectives, interventions, outcomes, and evaluation.

Provide an overview of the sources you reviewed while searching for an answer to your PICO question.

Overview of the Case
The collective bargaining between the company and the Local 384 Union has not been
successful for the last ten days. The impasse has been contributed by the union, which has been
asking a higher amount of the payment which the organization cannot afford to pay. The
workers demand the organization to pay them 10 cents higher per hour on top of what GMFC is
willing to offer. The management insists that some employees continue to co-paying for their
medical care. The union, on the other side, does not want to hear anything related to copayment.
The workers demand the organization to pay the full amount for their medical care. The
company is willing to offer only 5 cents per hour more.
At this point, the company cannot afford to pay the employees more than five-cent per
hour or even pay the full amount of medical care because it will have a negative implication on
the company (Fossum, 2015). Due to the unpredictability in the medical cost, in the country, the
organization cannot afford to cover the full payment for the employees’ medical insurance. It
will incur losses, and it will not be able to pay the employees’ wages. According to the financial
resource’s projections, the organization the company cannot go beyond what it is offering if at all,
it wants to remain in the market. Currently, the organization is experiencing reduced profits, and
the organization is planning to seek relief from the ever-increasing medical coverage.
Strategies that the Organization Should Implement

Therefore, I have a few recommendations on the strategy that the organization should
take. First, the company needs to exercise generosity and offer the workers a five-cent increase.
The organization management should summon the Local 384 Union leaders and make them
understand the reasons why the organization will not offer more than five cents increase per hour.
Also, the management should agree to offer free health coverage to the employees but only to the
diseases or injuries that have been caused by their direct contact with the company. As for the
rest of the health needs, the employees should agree to copay to a certain percentage.
Secondly, the management may reduce the percentage that the workers are required to
copay in their health coverage. For instance, instead of paying 50 percent of their out of pocket
money, they can contribute 40 percent for their health coverage to health needs that are not
attributable to the organization. The benefits are not bound to be changed after a certain time.
Lastly, the organization may opt lockout to pressure the union to accept the demand. A lockout
will apply in this case because the previous contract has already expired. A lockout should take
place for a period of one month to avoid more losses on the company. Although it will result in
the losses on the side of the organization, it will be an effective way of pressuring the union to
accept the contact as it is. To avoid wrong actions like strike, the company should guarantee the
employees job security upon their resumption.
Conclusion
It is imperative for both the organization and the Local 384 Union to be more lenient
about their demands. The management should be more generous to the workers by offering a
pay rise will be fair to the workers and will not hurt the organization. On the other side, the labor
union should not be adamant about what the company offers. It should agree to dialogue with
the company's bargaining team.
